Postoperative Care for Circumcision

Hello, doctor.
I am undergoing surgery with the second-generation circumcision device.
Since this is my first time changing the dressing myself, I noticed there is a complete ring of edema below the glans, and I cannot see the sutures.
Should I lift the edema to apply the medication directly on the sutures, or is it sufficient to apply it on the red area beneath the edema?

Huang, 20~29 year old female. Ask Date: 2023/07/07

Dr. Du Shixi reply Urology

Hello: If possible, it is better to gently open the wound to clean it properly.
If there is redness, it should also be cleaned.
If the area is too swollen to open, then it is best to keep it as clean and dry as possible.
Wishing you good health.

Post-circumcision care is crucial for ensuring proper healing and minimizing complications. After a circumcision, especially when performed with a device like the second-generation circumcision clamp, patients may experience swelling and discomfort in the area. Here’s how to manage swelling and apply medication effectively.

Understanding Swelling After Circumcision
Swelling is a common response following circumcision. It occurs as part of the body's natural healing process. The area around the glans (the head of the penis) may appear puffy, and this can sometimes obscure the surgical site, including any stitches or clips used during the procedure. It’s important to monitor the swelling and ensure it does not progress to a more serious condition, such as infection.

Medication Application
When it comes to applying medication, the primary goal is to ensure that the healing area is treated effectively while minimizing discomfort. Here are some steps to follow:
1. Clean the Area: Before applying any medication, gently clean the area with mild soap and water. Pat it dry with a clean towel. Avoid scrubbing, as this can irritate the sensitive skin.

2. Assess the Swelling: If the swelling is significant, it may be difficult to see the stitches or clips. However, you should not attempt to "flip up" the swollen area to expose the surgical site. Instead, focus on the areas that are visible and accessible.

3. Apply Medication:
- If your doctor has prescribed a topical antibiotic ointment, apply it gently to the areas that are red or irritated. This typically includes the skin around the glans and any visible stitches.

- Avoid applying medication directly on the swollen area unless instructed by your healthcare provider. The swelling itself may not require direct treatment, but the underlying skin should be cared for.

4. Monitor for Signs of Infection: Keep an eye out for increased redness, warmth, or discharge from the site, which could indicate an infection. If you notice any of these symptoms, contact your healthcare provider immediately.

5. Follow-Up Care: Attend any scheduled follow-up appointments with your healthcare provider. They will assess the healing process and provide additional guidance on care.

Managing Discomfort and Swelling
- Ice Packs: Applying a cold compress or ice pack wrapped in a cloth to the swollen area can help reduce swelling and alleviate discomfort. Limit application to 15-20 minutes at a time.

- Pain Management: Over-the-counter pain relievers such as acetaminophen or ibuprofen can help manage pain. Always follow the dosage instructions on the package or those provided by your doctor.

- Avoid Irritation: Wear loose-fitting underwear to avoid friction and irritation on the healing area.

Conclusion
In summary, after a circumcision, it is essential to manage swelling and apply medication correctly. Focus on cleaning the area, applying medication to visible irritated skin, and monitoring for any signs of complications. If you have any concerns about the healing process or the appearance of the surgical site, do not hesitate to reach out to your healthcare provider for personalized advice and support. Proper post-operative care is vital for a smooth recovery and to ensure the best possible outcome.
