Post-Surgery Complications: Swelling and Fluid Leakage After Hemorrhoid Ligation - Surgery

Swelling on the lateral side of the anus and tissue fluid exudation after sterilization surgery?

Hello, I would like to inquire about the following issues regarding swelling and pain on one side of the anus, as well as a protruding internal hemorrhoid, for which I am undergoing treatment.
This is my fourth follow-up visit for ligation treatment.
The procedure was very painful due to the location being close to the outer side, and the doctor administered anesthesia afterward.
The doctor mentioned that there were two areas bleeding, which were addressed during the procedure.
By the second day post-operation, I noticed that the rubber band had already fallen off, and today is the fourth day post-operation, during which I am still experiencing oozing of serous fluid and slight blood (requiring the use of a pad).
There is still swelling on one side of the anus, and there is visible white tissue at the anal area.
1.
Is it normal for the rubber band to fall off by the second day post-operation? In previous cases, it took about a week for it to fall off.
2.
Is the unilateral swelling due to the wound not healing yet? Approximately how many days will the swelling last? At what point should I return for a follow-up examination if the swelling persists?
3.
Is the oozing of serous fluid and blood also due to the wound not healing? This situation did not occur in previous ligation surgeries.
When should I return for a follow-up if this condition continues?
4.
Is the visible white tissue at the anus the internal hemorrhoid that was previously treated? Did it appear only after this surgery? Should I be concerned about this?
Thank you for your assistance with these questions.

1.
The duration for the rubber band to fall off can vary; if the ligation is shallow, it may fall off the next day.
2.
Swelling and pain may be due to the location being too lateral; if it does not improve after a week, a follow-up visit is necessary.
3.
It is likely an unhealed wound; if there is no improvement, it is important to have a physician examine it.

Post-surgery complications following hemorrhoid ligation can be concerning, and it’s important to address your questions with a clear understanding of the typical healing process and potential issues that may arise.

1. Rubber Band Ligation and Early Detachment: The fact that the rubber band fell off two days post-procedure may seem premature, especially if your previous experiences indicated a longer retention period. Typically, the rubber band is expected to remain in place for about a week before it falls off naturally as the tissue necroses and sloughs away. If it fell off too early, it could indicate that the tissue was not adequately secured or that the ligation was not as effective as intended. However, it’s also possible that the tissue was already sufficiently compromised, leading to an earlier detachment. Monitoring the area for any signs of excessive bleeding or infection is crucial.

2. Unilateral Swelling: The swelling on one side of the anus is likely due to the surgical trauma and the body’s inflammatory response to the procedure. Swelling can persist for several days to weeks, depending on individual healing rates and the extent of the procedure. Generally, you should expect some reduction in swelling within a week, but if the swelling persists beyond that or worsens, it would be advisable to return for a follow-up examination. Persistent swelling could indicate complications such as infection or hematoma formation.

3. Fluid Leakage and Blood: The ongoing leakage of serous fluid and light blood from the surgical site is often a sign of healing, but it can also indicate that the wound is not healing as expected. In the initial days following surgery, some drainage is normal, but if the amount increases or if you notice a change in color (such as bright red blood), it may warrant a visit to your healthcare provider. If this drainage continues beyond a week or is accompanied by increased pain, fever, or foul odor, you should seek medical attention promptly.

4. Visible White Tissue: The appearance of white tissue at the surgical site can be concerning. This tissue may represent necrotic tissue that is part of the healing process, or it could be a remnant of the previously treated hemorrhoid. If this tissue is not painful and does not show signs of infection (such as redness or discharge), it may simply be part of the healing process. However, if you have concerns about its appearance or if it changes in any way, it’s best to consult your physician for an evaluation.

In summary, while some swelling, drainage, and changes in the surgical site can be normal post-hemorrhoid ligation, it is essential to monitor your symptoms closely. If you experience increased pain, persistent swelling, excessive bleeding, or any signs of infection, do not hesitate to reach out to your healthcare provider. Regular follow-ups are crucial to ensure proper healing and to address any complications that may arise. Always prioritize your health and well-being by staying in communication with your medical team.
