Can the side effects of Seroquel occur after taking it for more than a year?

Hello Doctor, I am a patient with ankylosing spondylitis and have been taking Celebrex (celecoxib) twice a day for over a year.
Yesterday, I experienced tinnitus for about 3-4 hours, and today I feel dizzy (I feel dizzy while sitting, and also when I stand up; it’s not the kind of dizziness that occurs after sitting for a long time and then standing up).
This symptom has not occurred during the past year of taking Celebrex, only yesterday and today.
I would like to ask the doctor: the medication label for Celebrex mentions dizziness and tinnitus as side effects.
Is it possible for these symptoms to develop after taking the medication for over a year? What should I be aware of now? Thank you, Doctor.

Zhong, 20~29 year old female. Ask Date: 2023/07/12

Dr. Zeng Guosen reply Internal Medicine

The side effects of medications are rarely observed to occur after one year.
It is recommended to first consult an otolaryngologist for an examination.
If no issues are found and symptoms persist, it is advisable to return for a follow-up and discuss with a rheumatologist.
Thank you.

Salazopyrin, also known as sulfasalazine, is a medication commonly used to treat inflammatory conditions such as ankylosing spondylitis. While it is generally well-tolerated, like all medications, it can have side effects. Your question about whether side effects can develop after long-term use is quite pertinent, especially since you have been taking the medication for over a year.

To address your concern, it is indeed possible for side effects to develop even after prolonged use of a medication like Salazopyrin. The body can change over time, and individual responses to medications can vary. While some side effects may occur shortly after starting a medication, others can emerge after months or even years of treatment. This phenomenon is sometimes referred to as "delayed onset" side effects.
In your case, the symptoms of tinnitus (ear ringing) and dizziness that you are experiencing are noted side effects of Salazopyrin. Although these symptoms have not occurred during your first year of treatment, it is not uncommon for patients to report new side effects after long-term use. The exact mechanism behind this can vary; it could be related to changes in your body’s metabolism, interactions with other medications, or even the progression of your underlying condition.

When experiencing new symptoms, it is crucial to communicate with your healthcare provider. They can evaluate your symptoms in the context of your overall health and medication regimen. It is also important to consider other potential causes for your symptoms, such as dehydration, changes in blood pressure, or even other medications you may be taking.
In terms of what you should do next, here are some recommendations:
1. Consult Your Doctor: Schedule an appointment with your healthcare provider to discuss your symptoms. They may want to conduct a physical examination, review your medication list, and possibly run some tests to rule out other causes.

2. Monitor Symptoms: Keep a detailed record of your symptoms, including when they occur, their duration, and any other factors that may be relevant (such as changes in diet, activity level, or stress). This information can be invaluable for your healthcare provider.

3. Stay Hydrated: Ensure you are drinking enough water, as dehydration can contribute to dizziness and other symptoms.

4. Avoid Sudden Movements: Until you have a better understanding of your symptoms, try to avoid sudden changes in position, which can exacerbate dizziness.

5. Review Other Medications: If you are taking other medications, discuss with your doctor whether there could be any interactions contributing to your symptoms.

6. Consider Lifestyle Factors: Stress, fatigue, and lack of sleep can also contribute to dizziness and tinnitus. Evaluate your lifestyle and consider making adjustments if necessary.

In summary, while it is possible for side effects of Salazopyrin to develop after long-term use, it is essential to consult with your healthcare provider to determine the best course of action. They can help you assess whether your symptoms are related to the medication or if there may be other underlying issues that need to be addressed. Your health and well-being are paramount, and open communication with your healthcare team is key to managing your condition effectively.
