Could such blood values cause dizziness when getting up from a lying position?
Hello Doctor, my son is 20 years old.
Over the past year, he has had his blood drawn four times, with the following values: Hemoglobin (Hb) 12.6-13.6 g/dL, Red Blood Cell count (RBC) 4.5-4.7 million cells/µL, Platelet count (PLT) 220-250 thousand cells/µL, and White Blood Cell count (WBC) 6.0-8.0 thousand cells/µL.
He has reported that whenever he gets up from lying down, he often feels a bit dizzy (this started about six months ago).
It’s just a mild dizziness, not severe.
I have tried getting up from lying down myself, and I do not feel dizzy.
I would like to ask the doctor if, based solely on the blood values provided, I should suspect that my son's dizziness upon standing could be caused by anemia.
Thank you, Doctor.

Dr. Cai Dongqi reply Family Medicine
Hello, Hao: For mild anemia, you can consult a hematologist.
Based on the numbers, it may be necessary to diagnose whether there is thalassemia.

Dizziness upon standing, particularly after lying down, can be a concerning symptom, especially in a young adult like your son. While anemia can contribute to dizziness, it is essential to consider various factors that could be at play.
From the blood values you've provided, your son's hemoglobin (Hb) levels range from 12.6 to 13.6 g/dL, which are generally considered within the normal range for adult males (typically around 13.8 to 17.2 g/dL). His red blood cell (RBC) count also appears to be normal, as does his platelet count. These values suggest that he may not be significantly anemic, but it is crucial to consider the context of his symptoms.
Dizziness upon standing, known as orthostatic hypotension, occurs when there is a drop in blood pressure upon standing. This can happen for several reasons, including dehydration, prolonged bed rest, or autonomic nervous system dysfunction. In your son's case, if he has been lying down for extended periods, his body may not be adjusting quickly enough to the change in position, leading to a temporary drop in blood pressure and resulting in dizziness.
Additionally, other potential causes of dizziness should be considered. These include:
1. Dehydration: If your son is not drinking enough fluids, this can lead to decreased blood volume and contribute to dizziness.
2. Dietary Factors: A diet low in essential nutrients, particularly iron, vitamin B12, and folate, can lead to symptoms of anemia or contribute to feelings of dizziness.
3. Medication Side Effects: If your son is taking any medications, some can cause dizziness as a side effect, especially those that affect blood pressure.
4. Neurological Issues: Conditions affecting the inner ear or the brain can also cause dizziness. If the dizziness is accompanied by other symptoms, such as headaches, visual disturbances, or balance issues, further evaluation may be warranted.
5. Postural Changes: If your son is experiencing dizziness primarily when standing up after lying down, it could be a sign of orthostatic hypotension, which may not necessarily be related to anemia but rather to how his body is responding to changes in posture.
Given these considerations, it would be advisable for your son to consult with a healthcare provider. They may recommend further evaluation, including blood pressure measurements in different positions (lying down, sitting, and standing), additional blood tests to check for other potential causes of dizziness, and possibly a review of his diet and hydration status.
In summary, while anemia could potentially contribute to dizziness, the normal blood values suggest that it may not be the primary cause in your son's case. Other factors, particularly related to blood pressure regulation upon standing, should be explored. A thorough assessment by a healthcare professional will help determine the underlying cause and appropriate management for his symptoms.
