Ejaculatory pain
Since around June 26, I have experienced a sharp pain in my right thigh at the moment of ejaculation while masturbating.
I usually ejaculate multiple times in one session (not sure if this is normal, as I have phimosis), and each time I ejaculate, I feel pain.
I typically masturbate about twice a day, but recently I've been doing it once a day for the past week, and the area of sharp pain has actually increased.
Initially, the pain was localized to a small area at the base of my thigh, but now it has extended to half of my thigh.
I usually keep my phone and headphones in my right front pocket and use my right hand (not sure if this is related).
When I masturbate, I am usually only semi-erect, and I achieve full erection only at the moment of ejaculation (not sure if this is normal).
If I stop for a few seconds, I become flaccid again.
I have also experimented with stopping stimulation suddenly at the moment of ejaculation, and after the sensation subsides, I continue.
There was one instance where I pulled back my foreskin just before ejaculation, and since I have phimosis, the sensation immediately diminished, but with a little more stimulation, I ejaculated.
Lastly, I want to ask if ejaculating in a flow is problematic, and whether this could affect future fertility.

Dr. Li Xinyi reply Urology
It is recommended to consult a urologist regarding the issue of phimosis to determine if treatment is necessary, as improving hygiene may be more important.
This could potentially alleviate some of the nonspecific symptoms you described.

More Info
Ejaculation pain, or pain experienced during or after ejaculation, can be a concerning issue for many men. It can manifest in various ways, including sharp or stabbing sensations in different parts of the body, such as the thighs, as you described. Understanding the potential causes and implications of this pain is crucial for addressing the issue effectively.
Possible Causes of Ejaculation Pain
1. Nerve Irritation or Compression: The sensation of pain in the thigh during ejaculation could be related to nerve pathways. The pudendal nerve, which innervates the genital area, can sometimes be irritated or compressed, leading to referred pain in the thighs. This can occur due to prolonged sitting, certain sexual positions, or even tight clothing.
2. Pelvic Floor Muscle Tension: Tension in the pelvic floor muscles can also contribute to pain during ejaculation. If these muscles are overly tight or in spasm, they can create discomfort during sexual activity. This is often associated with anxiety or stress, which can lead to muscle tension.
3. Infections or Inflammation: Conditions such as prostatitis (inflammation of the prostate gland) or epididymitis (inflammation of the epididymis) can cause pain during ejaculation. These conditions may also present with other symptoms, such as discomfort in the pelvic area, urinary issues, or fever.
4. Anatomical Issues: Since you mentioned being uncircumcised (phimosis), this could potentially contribute to discomfort during ejaculation. If the foreskin is too tight, it may cause pain or discomfort during sexual activity, including masturbation.
5. Psychological Factors: Anxiety and stress can exacerbate physical symptoms. If you are worried about the pain or have had negative experiences related to sexual activity, this could lead to increased tension and discomfort.
Concerns Regarding Ejaculation Patterns
You mentioned that you experience multiple ejaculations in one session, which is not uncommon, but it can lead to increased sensitivity and discomfort. The fact that you experience pain only during ejaculation and not at other times is a key detail. However, if the pain persists or worsens, it is essential to consult a healthcare professional.
Impact on Fertility
Regarding your concern about whether the way you ejaculate (the flow of semen) could affect future fertility, it is generally not a concern unless there are underlying issues such as infections or anatomical problems. The mechanics of ejaculation, including how forcefully semen is expelled, do not typically impact sperm quality or fertility. However, if you are experiencing pain, it is advisable to seek medical advice to rule out any conditions that could affect reproductive health.
Recommendations
1. Consult a Healthcare Provider: Given the persistence and increase in pain, it is crucial to consult a urologist or a healthcare provider specializing in male reproductive health. They can perform a thorough evaluation, including a physical examination and possibly imaging studies, to determine the underlying cause of your pain.
2. Pelvic Floor Therapy: If muscle tension is suspected, pelvic floor physical therapy may be beneficial. A trained therapist can help you learn relaxation techniques and exercises to alleviate tension in the pelvic area.
3. Lifestyle Modifications: Consider evaluating your habits, such as prolonged sitting or tight clothing, which may contribute to nerve compression. Regular breaks and stretching can help alleviate discomfort.
4. Mental Health Support: If anxiety or stress is a contributing factor, speaking with a mental health professional may provide strategies to manage these feelings and reduce physical symptoms.
In summary, while ejaculation pain can be concerning, understanding the potential causes and seeking appropriate medical advice can help address the issue effectively. It is essential to prioritize your health and well-being by consulting with a healthcare professional who can provide personalized guidance based on your specific situation.
