Vomiting with brown, stringy material?

Hello Doctor: Since last night, I have been experiencing abdominal bloating and nausea, but I only vomited this morning.
After vomiting, I noticed that the yellow vomit contained brown, thread-like substances, and I am concerned it might be internal bleeding, so I would like to consult about it.
Additionally, I had some beef for dinner and am unsure if the thread-like substances are actually from the beef rather than blood.
Before vomiting, I had three bowel movements, and there was no blood in my stool.
Last night, I took the emergency contraceptive pill, as well as lflocin, cephalexin, moren, Gaviscon, and uropyridium.
Thank you for your response.

Dr. Lin Wufu reply Internal Medicine

Hello, it is difficult to confirm whether there is mild bleeding.
In fact, during vomiting, the esophagus can sometimes stretch forcefully, which may lead to tearing and result in mild bleeding.
If there is gastric bleeding, the vomit is typically tarry in appearance.
It is advisable not to worry too much if there is only one episode of vomiting, as it may just be a case of general gastroenteritis.
However, if vomiting occurs repeatedly, it is recommended to visit a family medicine or gastroenterology clinic for further evaluation and treatment.

Understanding the presence of brown stringy vomit can be concerning, especially when accompanied by symptoms such as abdominal bloating and nausea. The brown stringy material in your vomit could potentially be a few different things, and it’s important to consider the context of your recent dietary intake and any medications you may be taking.

Firstly, the brown color in vomit can sometimes be attributed to the presence of digested blood, which may indicate a gastrointestinal bleed. This is often referred to as "coffee ground" vomit due to its appearance, resembling coffee grounds. However, it is also possible that the brown stringy material is simply undigested food, particularly if you consumed red meat like beef, which can sometimes appear stringy when vomited.

Given that you mentioned you had three bowel movements prior to vomiting and did not notice any blood in your stool, this is a positive sign. It suggests that there may not be an active gastrointestinal bleed, as blood in the stool would typically accompany a significant bleed. However, the presence of brown stringy material in your vomit still warrants further investigation.

The medications you mentioned, including antibiotics (cephalexin), anti-inflammatory medications (like moren), and the use of a contraceptive pill (祈麗安), can sometimes cause gastrointestinal upset, leading to nausea and vomiting. Cephalexin, in particular, can disrupt the normal gut flora, potentially leading to gastrointestinal symptoms. If you have recently started any of these medications, it could be contributing to your symptoms.

In terms of immediate concerns, if you experience any of the following, it is crucial to seek medical attention promptly:
- Persistent vomiting
- Severe abdominal pain
- Signs of dehydration (such as dry mouth, decreased urination, or dizziness)
- Blood in your vomit or stool
- Any new or worsening symptoms
To further assess your condition, a healthcare provider may recommend diagnostic tests such as blood tests, imaging studies (like an abdominal ultrasound or CT scan), or an endoscopy to visualize the gastrointestinal tract. These tests can help determine the underlying cause of your symptoms and guide appropriate treatment.

In conclusion, while the presence of brown stringy vomit can be alarming, it is essential to consider the broader context of your symptoms, dietary habits, and medications. If your symptoms persist or worsen, do not hesitate to consult a healthcare professional for a thorough evaluation and management plan. Your health and well-being are paramount, and timely medical intervention can help address any potential issues effectively.
