Instilling artificial tears makes the eyes feel cool and soothing?

Hello doctor, I recently experienced discomfort in my eyes and visited an ophthalmologist.
The doctor diagnosed me with dry eyes and prescribed artificial tears (Pataday) for me.
I used them for a while without any issues.
However, yesterday after applying the artificial tears, I felt a cool sensation in my left eye, but not in my right eye.
I would like to know what this condition might be and if I need to schedule an immediate follow-up appointment.
Thank you, doctor.

Dr. Gao Pilin reply Ophthalmology

Hello: Different eyes have varying sensitivities, and sometimes, different eyes may react differently to medications or artificial tears.
This could explain why your left eye feels cool while your right eye does not.
If there is no discomfort, there is no need to schedule a follow-up appointment immediately!

When you experience a cool sensation in one eye after using artificial tears, it can be attributed to several factors related to the unique physiology of your eyes and the properties of the artificial tears themselves. It is not uncommon for individuals to notice differences in sensation between their eyes, and this can be influenced by various elements such as tear film stability, eye sensitivity, and even the formulation of the artificial tears used.

Firstly, the sensation of coolness in one eye could be due to the way the artificial tears interact with the surface of your eye. Artificial tears are designed to mimic natural tears, providing lubrication and moisture to alleviate dryness. Some formulations contain ingredients that may create a cooling effect upon application, which could explain why you feel a difference between your left and right eye. Additionally, the distribution of the drops may vary slightly between the two eyes, leading to a different sensory experience.

Another important consideration is the sensitivity of each eye. The cornea, which is the transparent front part of the eye, is densely packed with nerve endings. This means that even slight variations in moisture levels or the presence of foreign substances can lead to different sensations. If your left eye is more sensitive or has a different baseline level of hydration compared to your right eye, it may react differently to the artificial tears, resulting in the cool sensation you described.

Moreover, if you have a history of dry eye syndrome, it's possible that one eye may be more affected than the other. Dry eye can lead to an imbalance in the tear film, which can cause discomfort and varying sensations. If your left eye is drier or has a different tear film stability, it may respond more noticeably to the artificial tears, leading to the cooling sensation.

As for whether you need to return to your eye doctor, if the cool sensation is not accompanied by any pain, redness, or significant discomfort, it may not be necessary to seek immediate medical attention. However, if you experience any worsening symptoms, such as increased dryness, pain, or visual disturbances, it would be prudent to consult your eye care professional for further evaluation. They can assess your condition more thoroughly and determine if any adjustments to your treatment plan are needed.

In summary, the cool sensation in one eye after using artificial tears can be attributed to differences in eye sensitivity, the formulation of the artificial tears, and the individual tear film stability of each eye. If the sensation is mild and not associated with other troubling symptoms, it may not require immediate medical attention. However, maintaining open communication with your eye care provider is essential for managing your eye health effectively.
