Small red spots on the genital organs?

Hello, Dr.
Liu.
Recently, I have developed some dark red spots on my genital area, mostly located near the hair and primarily on the blood vessels.
They are neither painful nor itchy, but the areas that I have scratched do itch.
I visited the hospital, and the doctor mentioned that it could be due to external irritation or possibly pigment deposition caused by excessive moisture, and that it should gradually fade away.
However, I am still quite concerned since these spots appeared suddenly over the past three to four days.
Therefore, I would appreciate any advice you could provide.
Thank you for your response.

The health of the little brother is always a concern.
Those small red spots near the hair could be folliculitis.
If they are itchy, it might be eczema or a fungal infection.
In any case, it's best to consult a urologist!

More Info

The appearance of dark red spots on the genital area can be concerning, especially when they emerge suddenly. Based on your description, these spots are located near the hair follicles and are primarily on the blood vessels, with no associated pain or itching, except for areas that have been scratched. This presentation can be attributed to several potential causes, and understanding these can help alleviate your concerns.

Possible Causes
1. Folliculitis: This is an inflammation of the hair follicles, often caused by bacterial or fungal infections. It can present as red or dark red spots around hair follicles, and while it may not always be painful, scratching can lead to irritation and itching.

2. Allergic Reaction or Irritation: External factors such as soaps, lotions, or laundry detergents can cause contact dermatitis, leading to red spots. If you've recently changed any products, this could be a contributing factor.

3. Heat Rash: If the area is prone to sweating and is kept covered, heat rash can develop, resulting in red spots or bumps. This is particularly common in warm, humid environments.

4. Vascular Changes: The spots being on blood vessels suggests that they could be related to vascular changes, such as petechiae or purpura, which can occur due to minor trauma or pressure. These are usually harmless and may fade over time.

5. Pigmentation Changes: As mentioned by your doctor, prolonged irritation or pressure can lead to localized pigmentation changes, which may appear as dark red or brown spots.

6. Infections: Although less likely given the absence of pain or itching, sexually transmitted infections (STIs) can sometimes present with unusual skin changes. If there is any concern about potential exposure, it would be wise to get tested.

Recommendations
1. Avoid Scratching: Since scratching can exacerbate irritation and lead to further complications, try to avoid touching the affected areas. If itching occurs, consider using a mild, fragrance-free moisturizer to soothe the skin.

2. Monitor Changes: Keep an eye on the spots over the next few days. If they begin to change in appearance, increase in number, or if you develop any additional symptoms (such as pain, swelling, or discharge), seek medical attention promptly.

3. Maintain Hygiene: Ensure that the area is kept clean and dry. Use gentle, unscented soaps and avoid tight clothing that may cause friction.

4. Consult a Dermatologist: If the spots persist or worsen, consider consulting a dermatologist for a more thorough evaluation. They may perform a physical examination and, if necessary, a biopsy to rule out any underlying conditions.

5. Consider Lifestyle Factors: Reflect on any recent changes in your lifestyle that could contribute to skin changes, such as stress, diet, or changes in personal care products.

6. Follow-Up: If your doctor suggested that the spots would fade over time, ensure you follow up if they do not improve as expected. Persistent or worsening symptoms warrant further investigation.

In conclusion, while the appearance of dark red spots on the genitals can be alarming, many potential causes are benign and can resolve with time and proper care. However, staying vigilant and proactive about your health is essential, and seeking medical advice when in doubt is always a good practice.
