A small flesh-colored bump has appeared on the penis?

Hello Dr.
Li, due to my atopic dermatitis, I have developed warts in various places from scratching multiple times since childhood.
When my skin condition worsens, I even experience itching and skin breaks in the groin area, including the penis.
Recently, after scratching the skin on my penis, a small raised bump appeared near the base of the shaft.
I would like to ask for your opinion on whether this bump resembles a wart or molluscum contagiosum based on its appearance.
Thank you for your response.

When it comes to skin bumps on the penis, distinguishing between warts and molluscum contagiosum can be challenging, especially for individuals with a history of atopic dermatitis or eczema, as these conditions can lead to skin irritation and subsequent infections.
Warts, caused by the human papillomavirus (HPV), typically present as rough, raised bumps that can vary in size and may have a cauliflower-like appearance. They can be skin-colored or darker and are often found in clusters. Warts are generally painless but can become itchy or uncomfortable, especially if they are scratched or irritated.

On the other hand, molluscum contagiosum is a viral infection that results in small, flesh-colored or pearly bumps with a central dimple. These bumps are usually smooth and can appear anywhere on the body, including the genital area. Molluscum contagiosum is more common in children but can also affect adults, particularly those with compromised immune systems or skin conditions like eczema.

Given your history of atopic dermatitis and the recent scratching that led to a skin break, it is possible that the bump you are observing could be a result of irritation or a secondary infection. The skin's barrier function is often compromised in individuals with eczema, making them more susceptible to viral infections, including HPV and molluscum contagiosum.

To determine the exact nature of the bump, it is crucial to consult a healthcare professional, preferably a dermatologist. They can perform a physical examination and may take a biopsy if necessary to confirm the diagnosis. Self-diagnosing can lead to inappropriate treatment, which may exacerbate the condition.

In the meantime, it is advisable to avoid scratching the area to prevent further irritation or potential infection. Keeping the area clean and dry is essential, and using a gentle, fragrance-free moisturizer can help soothe the skin. If the bump becomes painful, red, or shows signs of infection (such as pus), seek medical attention promptly.

Regarding treatment, if the bump is confirmed to be a wart, options include topical treatments, cryotherapy (freezing), or laser therapy. For molluscum contagiosum, treatment may not be necessary as the condition often resolves on its own, but if it is bothersome, options include topical therapies or physical removal.

In summary, while both warts and molluscum contagiosum can present as bumps on the penis, their appearance and characteristics differ. Given your skin condition and the recent scratching, it is essential to consult a dermatologist for an accurate diagnosis and appropriate treatment. Avoiding irritation and maintaining skin hygiene will be beneficial in managing your symptoms until you can receive professional care.
