Penile Curvature: When to Seek Treatment or Surgery - Urology

Penile curvature

I have a question...
My penis bends when it is erect! The first 1-2 centimeters are straight, but the rest bends to the left at about a 45-degree angle! When it's not erect, the curvature is not very noticeable, just slightly bent, but it becomes very obvious when erect.
Also, I have a condition of phimosis, but I had a circumcision a few weeks ago.
Given the curvature of my penis, should I consider correction or surgery?

A Cheng, 10~19 year old female. Ask Date: 2004/01/12

Dr. Du Shixi reply Urology

Hello, a curved erection is not uncommon.
The exact angle of curvature should be evaluated by a physician.
If it interferes with sexual intercourse, corrective measures may be necessary.
I recommend consulting a urologist at a hospital.
Thank you.

Penile curvature, particularly when it becomes noticeable during an erection, can be a source of concern for many men. In your case, a curvature of approximately 45 degrees during erection, especially if it is accompanied by other conditions such as phimosis (tight foreskin), may warrant further evaluation and potential treatment.
The first step in addressing penile curvature is to understand its cause. In many cases, penile curvature can be attributed to a condition known as Peyronie's disease, which involves the development of fibrous scar tissue inside the penis, leading to curvature during erection. However, not all cases of curvature are due to Peyronie's disease; some men may have a congenital curvature that has been present since birth.
If the curvature is mild and does not cause pain or interfere with sexual function, treatment may not be necessary. However, if the curvature is significant enough to cause discomfort, pain, or difficulties during sexual intercourse, it is advisable to seek treatment. Here are some considerations regarding when to seek treatment or surgery:
1. Severity of Curvature: If the curvature is severe (typically over 30 degrees) and affects sexual performance or causes pain, it is a strong indication for treatment. In your case, a 45-degree curvature during erection is significant enough to consider intervention.

2. Associated Symptoms: If you experience pain during erections, difficulty achieving or maintaining an erection, or if the curvature is worsening, these are all signs that you should consult a healthcare professional.

3. Impact on Quality of Life: If the curvature affects your sexual relationships or causes psychological distress, seeking treatment can be beneficial. Sexual health is an important aspect of overall well-being, and addressing concerns can improve quality of life.

4. Consultation with a Specialist: A urologist specializing in male sexual health can provide a thorough evaluation. They may perform a physical examination and possibly imaging studies to assess the extent of the curvature and any underlying conditions.

5. Treatment Options: Treatment for penile curvature can vary based on the underlying cause and severity. Options may include:
- Medications: In some cases, medications may be prescribed to help reduce curvature or manage symptoms.

- Injections: Some treatments involve injecting medications directly into the scar tissue to help reduce curvature.

- Surgery: If conservative treatments are ineffective and the curvature is significant, surgical options may be considered. Surgical procedures can involve plication (folding the longer side of the penis), grafting (adding tissue to the shorter side), or penile prosthesis implantation in severe cases.

6. Post-Surgical Considerations: If surgery is performed, recovery time and post-operative care will be essential. Most men can return to normal activities within a few weeks, but sexual activity may need to be postponed for a period as advised by the surgeon.

In conclusion, given your description of a noticeable curvature during erection, it would be prudent to consult with a urologist. They can provide a comprehensive evaluation and discuss the best course of action tailored to your specific situation. Early intervention can often lead to better outcomes, so don’t hesitate to seek help. Remember, addressing these concerns is a positive step towards maintaining your sexual health and overall well-being.
