Blood Stains on Underwear and Occasional Pain: Urology Concerns - Urology

There are blood stains on the underwear at the glans area, and occasionally there is a sharp pain in the penis?

Hello doctor, I have recently noticed blood stains on my underwear for three consecutive days.
The amount of bleeding is not significant, and the size of the stains is about half the size of a fingernail.
The stains are located at the glans and meatus of the penis.
I can confirm that there is no external injury to my penis or glans.
Over the past few days, I have experienced occasional brief sharp pains in my penis, about twice a day, without any warning; it feels like being pricked by a needle.
However, after the sharp pain, there is no immediate bleeding.
Usually, I only notice the blood on my underwear several hours later when I go to the bathroom.
My last sexual intercourse was three days ago, during which I experienced the aforementioned sharp pain at the time of ejaculation, but it lasted for a long time.
Yesterday, during masturbation, there was also slight blood in the semen, but there was no pain during ejaculation, and urination is generally pain-free.
What could be the cause of this? Should I seek medical attention, or will it resolve on its own?

It sounds like you are experiencing some concerning symptoms related to your genital area, specifically blood stains on your underwear and occasional pain in the penis. Given the details you've provided, there are several potential causes for these symptoms, and it is important to approach this situation with caution.

First, the presence of blood in the urine or on the underwear, especially localized to the glans (the head of the penis), can indicate a few different issues. One possibility is that there may be a minor injury or irritation in the urethra, which is the tube that carries urine and semen out of the body. This could be due to friction during sexual activity or masturbation, especially if there was vigorous activity involved. The fact that you mentioned a brief stabbing pain could suggest some irritation or inflammation in the area.

Another potential cause could be a urinary tract infection (UTI) or a sexually transmitted infection (STI). While you mentioned that urination does not cause pain, some infections can present with intermittent symptoms, including blood in the urine or discharge. It’s also worth noting that certain conditions, such as prostatitis (inflammation of the prostate gland), can cause similar symptoms, including pain and blood in the semen.

Given that you have experienced blood during ejaculation and some pain, it is crucial to consider the possibility of a more serious underlying condition, such as a lesion or infection. While it is possible that these symptoms may resolve on their own, it is not advisable to wait and see without consulting a healthcare professional.
Here are some steps you should consider taking:
1. Seek Medical Attention: It is important to consult a urologist or your primary care physician as soon as possible. They can perform a thorough examination, which may include a physical exam, urinalysis, and possibly imaging studies or further tests to determine the cause of your symptoms.

2. Document Symptoms: Keep track of any additional symptoms you may experience, such as changes in urination, pain levels, or any new symptoms. This information can be helpful for your healthcare provider.

3. Avoid Irritation: Until you can see a doctor, try to avoid any activities that may further irritate the area, such as vigorous sexual activity or masturbation.

4. Stay Hydrated: Drinking plenty of fluids can help flush out your urinary system, which may help alleviate some symptoms if they are related to a UTI.

5. Follow-Up: If you notice any worsening of symptoms, such as increased pain, fever, or more significant bleeding, seek medical attention immediately.

In summary, while there may be benign explanations for your symptoms, the presence of blood and pain warrants a professional evaluation to rule out any serious conditions. Early diagnosis and treatment can help prevent complications and ensure your health and well-being. Please prioritize seeing a healthcare provider to address your concerns.
