Blood Stains on Underwear: Urological Concerns Explained - Urology

There are blood stains on the underwear?

Hello doctor, I have recently noticed blood stains on my underwear for three consecutive days.
The amount of bleeding is not significant, and the size of the stains is about half the size of a fingernail.
The stains are located at the glans and the urethral opening.
I can confirm that there are no external injuries to my penis or glans.
Over the past few days, I have experienced occasional very brief sharp pains in my penis, about twice a day, without any warning; it feels like being pricked by a needle.
However, after the sharp pain, there is no immediate bleeding.
It usually takes several hours after urinating to notice blood on my underwear.
My last sexual intercourse was three days ago, during which I experienced the aforementioned sharp pain at the time of ejaculation, but it lasted for a long time.
Yesterday, during masturbation, my ejaculate also had slight blood stains, but there was no pain during ejaculation, and I do not experience pain when urinating.
What could be the cause of this? Should I seek medical attention, or will it resolve on its own?

Blood stains on underwear, particularly when localized to the glans penis and accompanied by intermittent sharp pain, can be concerning and warrant further investigation. The symptoms you describe, including the presence of blood and the brief episodes of sharp pain, could be indicative of several underlying urological conditions.

One potential cause of blood in the urine or on the underwear could be a urinary tract infection (UTI) or a sexually transmitted infection (STI). Infections can lead to inflammation and irritation of the urethra, which may cause bleeding, especially during ejaculation or urination. The fact that you experienced a sharp pain during ejaculation could suggest that there is some irritation or inflammation in the urethra or surrounding tissues.

Another possibility is that you may be experiencing a condition known as "hematospermia," which is the presence of blood in the semen. This can occur due to various reasons, including inflammation of the prostate (prostatitis), seminal vesiculitis, or even trauma to the reproductive organs. While it can be alarming, hematospermia is often benign, especially in younger individuals, but it should not be ignored.

Additionally, the sharp pain you are experiencing could be related to a condition known as "urethral stricture," where the urethra becomes narrowed, leading to pain and potential bleeding. This can occur due to previous infections, trauma, or other underlying conditions.

Given that you mentioned the blood is not accompanied by pain during urination and that the bleeding is relatively minor, it may not be an emergency situation. However, it is essential to consult a healthcare professional for a thorough evaluation. A urologist can perform a physical examination, possibly including a urinalysis and imaging studies, to determine the exact cause of your symptoms.

In the meantime, here are some general recommendations:
1. Hydration: Drink plenty of fluids to help flush out your urinary system. This can help reduce irritation and may alleviate some symptoms.

2. Avoid Irritants: Refrain from using any harsh soaps or products on the genital area that could cause irritation.
3. Monitor Symptoms: Keep track of any changes in your symptoms, such as increased pain, changes in urination patterns, or the presence of more blood.

4. Avoid Sexual Activity: Until you have a clearer understanding of what is causing the bleeding, it may be wise to avoid sexual activity to prevent further irritation or injury.

5. Seek Medical Attention: If the bleeding persists, worsens, or if you develop additional symptoms such as fever, chills, or significant pain, seek medical attention promptly.

In summary, while the symptoms you are experiencing may not indicate a severe condition, they should not be ignored. A healthcare professional can provide a proper diagnosis and treatment plan tailored to your specific situation. Early intervention can help prevent potential complications and ensure your overall urological health.
