Missed taking emergency contraception pills for two days (within 72 hours)?

Hello Doctor: This month marks my second month taking Aygestin.
I was unable to take my medication on September 28 and 29 due to a trip, and I couldn't purchase it, so I missed those doses.
My last dose was on the 27th, and I usually take it at 10:30 PM every day.
I was able to resume taking the medication on the afternoon of September 30.
Is it still effective to take the missed doses, or should I stop the medication and restart it after 7 days? Additionally, I had unprotected intercourse with ejaculation on the first day after my period ended on September 22.
Is there a possibility of pregnancy? Thank you, Doctor, for taking the time to answer.

Cc, 20~29 year old female. Ask Date: 2023/09/30

Hello, the oral contraceptive pill (COC) must be taken at the same time every day to achieve a 99% effectiveness in preventing pregnancy.
If you miss a dose for more than 24 hours, you should stop taking the current pack of pills and refrain from taking them for seven days (including the days you missed).
After that, you can continue with the next cycle.
If you miss a pill and do not experience the expected withdrawal bleeding during the subsequent pill-free interval, you should suspect the possibility of pregnancy.
Thank you! Wishing you peace and health.

When it comes to missed birth control pills, it's important to understand how oral contraceptives work and what steps to take if you've missed doses. In your case, you missed two consecutive days of taking your birth control pills, which can potentially affect your contraceptive effectiveness.

First, let's clarify the situation: you last took your pill on September 27 and missed the pills on September 28 and 29. You resumed taking the pill on September 30. According to the guidelines for combination oral contraceptives, if you miss one pill, you should take it as soon as you remember, and then continue taking the rest of your pills at the usual time. If you miss two pills in a row during the first two weeks of your pack, you should take the most recent missed pill as soon as you remember, discard the earlier missed pill, and continue taking your pills as scheduled. However, you should also use a backup method of contraception, such as condoms, for the next seven days to ensure protection.

Since you had unprotected intercourse on September 22, which was just before you missed your pills, there is a possibility of pregnancy. The effectiveness of the pill relies on consistent daily intake, and missing doses can lead to ovulation. If you are concerned about the possibility of pregnancy, you might consider taking a home pregnancy test if your period is late or consult with your healthcare provider for further evaluation.

In summary, here are the steps you should take:
1. Resume Taking Your Pills: Since you have already resumed taking your pills on September 30, continue taking them as directed.

2. Use Backup Contraception: Since you missed two pills, use a backup method of contraception for at least the next seven days.

3. Monitor for Pregnancy: If your period is late or if you experience any unusual symptoms, consider taking a pregnancy test or consulting your healthcare provider.

4. Consult Your Doctor: If you have any further questions or concerns about your birth control method or potential pregnancy, it’s always a good idea to reach out to your healthcare provider for personalized advice.

In the future, to avoid missing pills, consider setting reminders on your phone or using a pill organizer. Consistency is key with oral contraceptives to maintain their effectiveness in preventing pregnancy. If you find that you frequently forget to take your pills, you might want to discuss alternative contraceptive methods with your healthcare provider, such as the patch, ring, or long-acting reversible contraceptives like IUDs, which do not require daily attention.
