Fluctuating flashes of light in the visual field?

Hello Doctor: A few days ago, I woke up in the middle of the night to use the restroom and suddenly noticed a flash at the edges of my vision.
I thought it was just a trick of the light or due to blinking.
After returning to my room, my vision started to flicker continuously, resembling rapid blinking or a flickering light bulb, even though the room was very dark with almost no light.
The flickering continued even with my eyes closed.
Today, while lying in bed and closing my eyes for a while, I experienced the same issue upon opening my eyes, although it was less severe.
When I covered my eyes and then opened them, the flickering stopped, and it didn't persist in the dark.
I later tried a few times and found that if I kept my eyes closed long enough before opening them, a similar situation would occur (though I’m not sure if it was due to the bright overhead lights when I opened my eyes), or if I kept my eyes open without focusing for a long time, it would also happen, but it would stop once I focused on something.
Could you please explain what possible causes might lead to this condition? Is it serious? I have approximately 200 degrees of myopia in both eyes and a bit of astigmatism, and my intraocular pressure has been slightly higher than average since childhood.
Thank you, Doctor.

Hello: There could be multiple potential causes for this situation.
Here are a few possible reasons: 1.
Macular diseases: Certain macular conditions, such as macular degeneration, may lead to visual disturbances or flickering, especially in low-light environments.
2.
Eye strain: Prolonged periods of closing the eyes or staring at a specific point can result in eye fatigue, which may cause some visual discomfort.
3.
Eye adaptation issues: Sometimes, when transitioning between different environments or lighting conditions, the eyes may require time to adjust.
4.
Light stimulation: Sudden exposure to bright light or intense lighting conditions can cause temporary visual flickering.
Given the variety of potential causes for this situation, I recommend that you visit an ophthalmologist for a thorough evaluation and examination to better understand your condition.

Flickering vision, or the sensation of seeing flashes or flickers of light, can be concerning and may have various underlying causes. Based on your description, it seems that you are experiencing intermittent visual disturbances, particularly in low-light conditions. This phenomenon can be attributed to several factors, and understanding these can help alleviate your concerns.

Firstly, one common cause of flickering vision is related to the vitreous gel in the eye. As we age, the vitreous can undergo changes, leading to the formation of floaters or flashes of light. These flashes occur when the vitreous pulls on the retina, which can create the sensation of flickering. This is often benign, especially if there are no accompanying symptoms such as a sudden increase in floaters or a shadow in your vision.

Another potential cause could be related to the retina itself. Conditions such as retinal detachment or tears can lead to similar visual disturbances. However, these conditions are usually accompanied by other symptoms, such as a sudden increase in floaters, a curtain-like shadow over your vision, or significant changes in your visual field. Given that you have not reported these additional symptoms, it may be less likely that your flickering vision is due to a serious retinal issue.

Additionally, your experience of flickering vision when transitioning from darkness to light could be related to the way your eyes adjust to changes in lighting conditions. When you close your eyes for an extended period and then open them, your eyes may take a moment to adjust to the ambient light, which can create a temporary visual disturbance. This is often a normal response and may not indicate any underlying pathology.

Given your history of myopia (nearsightedness) and slightly elevated intraocular pressure, it is essential to monitor your eye health closely. While elevated eye pressure can be a risk factor for glaucoma, regular eye examinations can help detect any changes early. If your eye pressure is being managed and you are under the care of an eye specialist, this should provide some reassurance.

In terms of management, it is crucial to maintain regular eye check-ups, especially since you have a history of myopia and elevated eye pressure. If the flickering persists or worsens, or if you experience any new symptoms, it would be advisable to seek further evaluation from an eye care professional. They may perform a comprehensive eye examination, including a dilated fundus exam, to assess the health of your retina and vitreous.

In conclusion, while flickering vision can be alarming, it is often benign, particularly in the absence of other concerning symptoms. However, given your specific eye health history, maintaining regular follow-ups with your eye care provider is essential. They can provide personalized advice and ensure that any potential issues are addressed promptly. If you have any further questions or concerns, do not hesitate to reach out to your healthcare provider for guidance.
