Post-operative issues following circumcision?

Currently, it has been about 9 to 10 days post-surgery, and the sutures fell out three days ago due to frequent erections (I am very prone to reactions).
The process involved severe pain, and there is a significant difference in pain sensation on either side of the wound (the skin tends to retract inward, making it difficult to care for the wound, and pulling it open causes extreme pain).
The left side has a faint scar, and upon inspection, there is dark red tissue inside, while the right side and the ligament area are bright red, especially the right wound.
Opening it up causes intense pain due to contact with the antibiotic ointment.
Currently, I am taking Ketorolac, but it does not alleviate the pain, which also radiates to the testicular area, severely affecting my daily life and routine.
I initially thought the pain was due to the sutures, but upon counting, they seem to be intact, and the adhesive strip is also a complete circle.
The current situation is that only the right side has intense pain; other areas only hurt upon touch.
The only relief method is to shower, allowing the ointment to wash away slightly, and then drying it, which provides temporary relief.
Should I return for a follow-up appointment, or should I wait for the wound to heal a bit more? I am very concerned that frequent erections may have caused the sutures to pull and the wound to reopen, requiring stitches again, or that there may be an infection.

Hello: If you still experience pain after taking medication, it is recommended to return for a follow-up consultation.
In cases of inflammation, there is often redness.
Wishing you good health.

Post-circumcision complications can be quite distressing, particularly when pain and recovery issues arise. It sounds like you are experiencing significant discomfort and concerns regarding the healing process after your circumcision. Let’s break down the potential issues you may be facing and explore some management strategies.

Firstly, it’s important to understand that after a circumcision, the healing process can vary significantly from person to person. Typically, the initial healing phase lasts about 1 to 2 weeks, during which time you may experience swelling, redness, and discomfort. However, if you are experiencing severe pain, especially if it radiates to other areas such as the testicles, this could indicate a complication that needs to be addressed.

The fact that you mentioned the stitches (or staples) have fallen out prematurely could be a concern. Stitches are meant to hold the skin together while it heals, and if they come out too early, it can lead to improper healing or even reopening of the wound. The pain you’re experiencing, particularly on the right side, could be due to several factors:
1. Infection: If the wound is bright red and painful, it could be a sign of infection. Infections can cause increased pain, swelling, and discharge. If you notice any pus or an unusual odor, it’s crucial to seek medical attention immediately.

2. Inflammation: The bright red tissue you described may indicate inflammation, which is a normal part of the healing process but can sometimes become excessive. This can cause significant discomfort and may require treatment.

3. Nerve Pain: The pain radiating to the testicles could be a result of nerve irritation or damage during the procedure. This type of pain can sometimes be challenging to manage and may require specific medications or therapies.

4. Tension on the Wound: Frequent erections can indeed put tension on the healing tissue, potentially causing pain and discomfort. If the skin is pulling or stretching too much, it may lead to increased pain and could hinder the healing process.

Given your symptoms, it is advisable to return to your healthcare provider for a follow-up appointment. They can assess the wound, check for signs of infection, and determine if any additional treatment is necessary. It’s essential to communicate all your symptoms, including the severity of the pain and any changes in the appearance of the wound.

In the meantime, here are some general recommendations to help manage your pain and promote healing:
- Pain Management: Continue taking prescribed pain medications as directed. If you find that they are not effective, discuss this with your doctor. They may adjust your dosage or prescribe a different medication.

- Wound Care: Keep the area clean and dry. Follow your doctor’s instructions regarding the use of topical ointments or dressings. Avoid any products that may irritate the wound.

- Avoid Irritation: Try to minimize activities that may cause friction or tension on the area, including vigorous physical activity or sexual activity, until you have been cleared by your doctor.

- Cold Compress: Applying a cold compress to the area may help reduce swelling and numb the pain temporarily.

- Monitor Symptoms: Keep an eye on the wound for any changes, such as increased redness, swelling, or discharge. If you notice any concerning symptoms, seek medical attention promptly.

In conclusion, while some discomfort is expected after a circumcision, severe pain, especially if it is worsening or accompanied by other symptoms, should not be ignored. A follow-up visit with your healthcare provider is essential to ensure proper healing and to address any complications that may have arisen. Your health and comfort are paramount, and timely intervention can help prevent further issues.
