Itchy and painful skin?
Hello Doctor: Recently, I have been experiencing skin pain (only when I am in bed) that feels like being bitten by something, followed by itching.
The itching lasts for about 5 to 30 minutes, and there is no redness or swelling.
If I scratch it continuously, a small hole (less than 1mm) appears, but that only happens if I keep scratching.
I'm not sure if it's an insect bite or if I've just scratched my skin.
I visited a dermatologist, and the doctor suggested it might be an insect bite, but was unsure of the type of insect.
I have washed and changed my mattress and bedding.
I also checked the surrounding area and didn't see any signs of insects (though they could be too small to notice).
Could you please clarify what type of insect it might be, or could it just be an allergy? Thank you for your help.

More Info
Understanding skin itch and pain can be quite complex, especially when symptoms are localized and intermittent, as you described. Based on your account, it seems you are experiencing discomfort primarily while in bed, which raises several possibilities regarding the underlying causes.
Possible Causes
1. Insect Bites: The sensation of pain and itching that you experience could indeed be due to insect bites. Common culprits include bed bugs, fleas, or even mosquitoes. Bed bugs, in particular, are notorious for causing itchy welts that can appear after a night’s sleep. They tend to hide in the seams of mattresses and bedding, making them difficult to detect. If you suspect bed bugs, consider using a magnifying glass to inspect your mattress and bedding closely, or even consult a pest control professional.
2. Allergic Reactions: Another possibility is that you are experiencing an allergic reaction to something in your bedding or mattress. This could be due to dust mites, which are microscopic organisms that thrive in bedding and can cause allergic reactions in sensitive individuals. Symptoms of dust mite allergies can include itching, sneezing, and skin irritation. Washing your bedding in hot water and using allergen-proof covers for pillows and mattresses can help reduce exposure.
3. Skin Conditions: Conditions such as eczema or contact dermatitis could also be responsible for your symptoms. These conditions can cause the skin to become itchy and inflamed, especially in response to irritants or allergens. If you have a history of skin conditions, this could be a contributing factor.
4. Environmental Factors: Sometimes, environmental factors such as dry air or irritants in your bedroom (like certain fabrics or cleaning products) can lead to skin irritation. If your skin is particularly sensitive, consider using a humidifier to maintain moisture in the air and avoid harsh detergents when washing your bedding.
5. Psychogenic Itch: In some cases, psychological factors can lead to sensations of itchiness or discomfort. Stress and anxiety can manifest physically, leading to sensations that may not have a clear physical cause. If you find that your symptoms worsen during stressful times, this could be a contributing factor.
Solutions
1. Inspect and Clean: Thoroughly inspect your bedding and mattress for signs of insects. Wash all bedding in hot water and consider using a vacuum cleaner with a HEPA filter to remove dust mites and other allergens.
2. Use Protective Covers: Invest in dust mite-proof covers for your pillows and mattress. These can significantly reduce exposure to allergens.
3. Moisturize: If your skin is dry, applying a fragrance-free moisturizer can help alleviate itching. Look for products that are specifically designed for sensitive skin.
4. Consult a Dermatologist: If symptoms persist, it would be wise to consult a dermatologist. They can provide a thorough examination and may recommend allergy testing or prescribe topical treatments to alleviate your symptoms.
5. Monitor Your Environment: Pay attention to any changes in your environment that coincide with your symptoms. This could include new cleaning products, changes in your bedding, or even dietary changes.
6. Stress Management: If you suspect that stress may be contributing to your symptoms, consider incorporating stress-reducing practices into your routine, such as mindfulness, yoga, or regular exercise.
In conclusion, while it can be challenging to pinpoint the exact cause of your skin discomfort, considering the factors mentioned above can help guide you toward finding relief. If your symptoms continue or worsen, seeking professional medical advice is essential to ensure proper diagnosis and treatment.
