Itchy foreskin?

Hello, doctor.
Recently, I've been experiencing itching inside the foreskin.
I clean it daily and there is no smegma, but lately, due to the itching, I've noticed some white flakes when I rub it.
There is no pain or swelling, just the itching, and I haven't had any sexual intercourse.
Could this be a fungal infection of the foreskin? If so, what topical creams are available at the pharmacy that I can use?

Tim, 20~29 year old female. Ask Date: 2023/11/21

More Info

Itchy foreskin can be a common issue and may arise from various causes. The symptoms you describe, including itching and the presence of white flakes, suggest a possible dermatological or infectious condition. Here are some potential causes and treatment options:

Possible Causes
1. Fungal Infections: One of the most common causes of itching in the genital area, including the foreskin, is a fungal infection, such as candidiasis (yeast infection). This condition can occur even in the absence of sexual activity, especially if there are factors like moisture, warmth, or an imbalance in the normal flora.

2. Balanitis: This is an inflammation of the glans (the head of the penis) and can also affect the foreskin. It can be caused by irritants, infections (bacterial or fungal), or poor hygiene. Symptoms often include redness, swelling, and itching.

3. Contact Dermatitis: This condition results from an allergic reaction to soaps, lotions, or other products that come into contact with the genital area. It can lead to itching and irritation.

4. Psoriasis or Eczema: These chronic skin conditions can also affect the genital area, leading to dry, itchy skin and flaking.

5. Sexually Transmitted Infections (STIs): While you mentioned no recent sexual activity, it's important to consider that some STIs can cause itching and irritation.

Treatment Options
1. Antifungal Creams: If a fungal infection is suspected, over-the-counter antifungal creams such as clotrimazole (Lotrimin) or miconazole (Monistat) can be effective. These creams are typically applied to the affected area as directed.

2. Hydrocortisone Cream: For inflammation and itching, a mild hydrocortisone cream may help reduce symptoms. However, it should be used sparingly and not for prolonged periods without medical advice.

3. Proper Hygiene: Maintaining good hygiene is crucial. Gently cleaning the area with mild soap and water can help prevent irritation. Ensure the area is thoroughly dried after washing.

4. Avoid Irritants: If you suspect contact dermatitis, try to identify and avoid any potential irritants, such as scented soaps or lotions.

5. Consult a Healthcare Provider: If symptoms persist or worsen, it is essential to consult a healthcare provider for a proper diagnosis and treatment plan. They may perform a physical examination and, if necessary, take a sample for laboratory testing to identify any underlying infections.

When to Seek Medical Attention
- If the itching is severe and persistent.

- If you notice any unusual discharge, swelling, or pain.

- If over-the-counter treatments do not provide relief after a few days.

In conclusion, while your symptoms may suggest a fungal infection or another dermatological issue, a proper diagnosis from a healthcare professional is essential for effective treatment. Over-the-counter antifungal creams may be a good starting point, but if symptoms persist, further evaluation is necessary to rule out other conditions.
