Abnormal bleeding
Hello Doctor, I started taking the oral contraceptive pill (Diane) in April of this year for menstrual regulation and contraception.
In November, I took antibiotics for two to three weeks due to a urinary tract infection.
At the end of November (while still on the contraceptive pill), I experienced slight abnormal bleeding, with a couple of small blood streaks and clots, which occurred twice and were noticed during urination.
On November 30, I went to a major hospital's obstetrics and gynecology department for a transvaginal ultrasound, and the results were normal.
The doctor suggested that it might be due to stress and hormonal imbalance, and prescribed me five days' worth of Metformin.
On December 9, the fifth day after stopping the contraceptive pill, I experienced withdrawal bleeding, but even on the eighth day of bleeding, December 16, I still felt some heaviness in my abdomen, and occasionally noticed a little blood during urination.
Is this considered abnormal bleeding? Since I had a transvaginal ultrasound done half a month ago, do I need to have another examination? I am worried there might be an issue with my endometrium...
I have a history of depression and have been taking antidepressants for two to three years.
In the past couple of months, I have often felt anxious and had trouble sleeping due to physical discomfort.
I am overweight and was diagnosed with polycystic ovary syndrome several years ago.
Thank you, Doctor!

Abnormal bleeding can be a concerning issue for many women, especially when they are using hormonal birth control methods such as the pill. In your case, you have been taking the birth control pill (Diane) since April and have recently experienced some irregular bleeding after a course of antibiotics for a urinary tract infection. It’s important to understand the potential causes of abnormal bleeding while on birth control and when to seek further medical evaluation.
Firstly, hormonal contraceptives can sometimes lead to irregular bleeding, particularly during the first few months of use. This is often referred to as "breakthrough bleeding." It can occur due to the hormonal fluctuations that the body is adjusting to. In your case, the slight bleeding you experienced in late November could be attributed to this phenomenon, especially since you mentioned that it was minor and only occurred during urination.
The fact that you had a vaginal ultrasound that showed no abnormalities is reassuring. However, it is also essential to consider other factors that could contribute to your symptoms. Stress and hormonal imbalances can significantly affect your menstrual cycle and overall reproductive health. You mentioned experiencing stress and anxiety, which can lead to changes in your menstrual pattern. Additionally, your history of polycystic ovary syndrome (PCOS) can also play a role in irregular bleeding, as this condition is associated with hormonal imbalances.
Regarding your concerns about the bleeding persisting for several days and the abdominal discomfort, it is advisable to monitor your symptoms closely. If the bleeding continues, becomes heavier, or is accompanied by severe pain, it would be prudent to seek further medical evaluation. While the ultrasound results were normal, persistent symptoms could warrant additional testing, such as a pelvic exam or further imaging, to rule out any underlying conditions like endometrial issues or ovarian cysts.
It’s also worth noting that certain medications, including antidepressants, can have side effects that may influence your menstrual cycle. If you have been feeling particularly anxious or if your mental health has been affected by your physical symptoms, it might be beneficial to discuss this with your healthcare provider. They can help you manage both your physical and mental health concerns, which are often interconnected.
In summary, while some irregular bleeding can be normal when starting or adjusting to hormonal birth control, it is essential to remain vigilant about your symptoms. Given your history of PCOS, anxiety, and the recent use of antibiotics, it’s crucial to maintain open communication with your healthcare provider. If your symptoms persist or worsen, do not hesitate to seek further evaluation to ensure your reproductive health is being appropriately managed. Remember, your health and well-being are paramount, and addressing any concerns early can lead to better outcomes.
