Internal bleeding after a lower abdominal impact?

Hello doctor, I would like to ask a question.
A week ago, I was involved in a car accident and sustained minor injuries.
At the time of the accident, I felt a impact on my lower abdomen, but since I didn't feel anything afterward, I didn't pay much attention to it.
A few days after the accident, I masturbated and felt fine.
However, today while masturbating, I noticed some pain and found a small area of deep red near the base of my penis, which I suspect is internal bleeding.
Should I observe this for a few days, or should I seek medical attention? Additionally, why did I not experience any issues immediately after the accident or a few days later, but now, a week later, I am feeling pain?

Dr. Li Minghui reply Urology

Hello: If there is pain, you should seek medical attention.
Generally, bruising appears later, but pain should occur at the time of injury and gradually improve.
Wishing you good health.

Experiencing abdominal trauma, especially after a car accident, can lead to various complications, including internal bleeding. It’s crucial to understand the signs and symptoms of internal bleeding and when to seek medical attention. Given your situation, where you experienced a collision that resulted in abdominal impact, it's essential to monitor your condition closely.

Internal bleeding can occur without immediate symptoms. In some cases, the body may initially compensate for the blood loss, and symptoms may not manifest until days later. This delayed response can be attributed to several factors, including the location and severity of the injury, the amount of blood lost, and individual physiological responses. For instance, if the injury affected blood vessels or organs, bleeding may not be significant enough to cause immediate distress but can accumulate over time, leading to pain or other symptoms.

In your case, the pain you are experiencing a week after the accident, particularly in the area near the base of the penis, could be indicative of several issues. It might be related to the trauma itself, such as bruising or swelling in the surrounding tissues, or it could be a sign of internal bleeding. The deep red coloration you observed could suggest bruising, which is often a result of bleeding under the skin. However, it’s essential to differentiate between a benign bruise and a more serious condition that requires medical intervention.

Here are some signs that warrant immediate medical attention:
1. Increased Pain: If the pain worsens or becomes severe, it’s crucial to seek medical help.

2. Swelling or Bruising: Significant swelling or extensive bruising in the area can indicate internal bleeding.

3. Changes in Urination: Any changes in urination, such as blood in the urine or difficulty urinating, should be evaluated by a healthcare professional.

4. Dizziness or Fainting: These symptoms could indicate significant blood loss and require urgent care.

5. Abdominal Distension: If your abdomen becomes hard or distended, this could be a sign of internal bleeding or other complications.

Given that you experienced a trauma and are now noticing pain and discoloration, it would be prudent to consult a healthcare provider. They may recommend imaging studies, such as an ultrasound or CT scan, to assess for any internal bleeding or organ damage.
In terms of your question about why symptoms may appear later, it is not uncommon for injuries to manifest symptoms days after the initial trauma. This can be due to the gradual accumulation of blood in the abdominal cavity or the development of inflammation in response to the injury.
In conclusion, while it may be tempting to wait and see if the symptoms resolve, it is always better to err on the side of caution, especially after experiencing trauma. Seeking medical evaluation can provide peace of mind and ensure that any potential complications are addressed promptly. Remember, your health and safety are paramount, and it’s always better to be proactive when it comes to potential internal injuries.
