Postoperative conditions after anal fistula surgery?
Hello Doctor: I underwent anal fistula surgery on October 26.
The wound has been healing, but there is still a small opening that intermittently bleeds (deep red, sometimes with a small amount of yellow-green pus).
I had a follow-up appointment at the end of November, and the doctor examined the opening, indicating that it has a base and is not a recurrence, and that it should heal soon.
However, it has now been another month, and the small opening has not disappeared.
I am wondering if this could be a sign of recurrence? Thank you for taking the time to respond! Here is the link to the wound photo: https://imgus.cc/Xqn3P.

Dr. Zhang Shuhao reply Surgery
The wound has a small opening and there is purulent bloody discharge, which raises the possibility of recurrence or an incompletely cleared fistula.
If the surgeon does not consider it to be a recurrence, the clinical judgment of the physician should be prioritized.
If there are symptoms such as swelling, pain, discomfort, or fever, a follow-up appointment for examination is still necessary.

Post-surgery concerns, particularly regarding anal fistula healing, can be quite complex and often require careful monitoring and management. After undergoing anal fistula surgery, it is not uncommon for patients to experience some degree of drainage from the surgical site. However, the presence of a small hole that intermittently bleeds and produces pus can raise concerns about the healing process and potential complications.
Firstly, it is essential to understand that the healing of an anal fistula can take time. The surgical procedure aims to remove the fistula and promote healing of the surrounding tissue. However, the healing process can vary significantly from one individual to another, depending on factors such as the extent of the fistula, the patient's overall health, and adherence to post-operative care instructions.
The fact that your doctor has assessed the small hole and indicated that it has a bottom and is not a recurrence of the fistula is a positive sign. This suggests that the surgical site is still in the healing phase. However, the ongoing drainage, especially if it is deep red and occasionally yellow-green, may indicate that there is still some inflammation or infection present. It is crucial to monitor the characteristics of the drainage. If the drainage becomes more profuse, changes in color, or is accompanied by increased pain, fever, or other systemic symptoms, it may warrant further evaluation.
In terms of complications, while recurrence of the fistula is a concern, other issues such as infection or abscess formation can also occur. An infection can lead to increased drainage and may require additional treatment, such as antibiotics or, in some cases, further surgical intervention. It is also important to maintain good hygiene in the area to prevent infection and promote healing.
If the small hole persists beyond the expected healing time, it is advisable to follow up with your healthcare provider. They may consider additional imaging studies, such as an MRI or ultrasound, to assess the area more thoroughly. This can help determine if there are any underlying issues that need to be addressed.
In summary, while some drainage and a small hole can be part of the normal healing process after anal fistula surgery, persistent symptoms should not be ignored. Regular follow-up with your healthcare provider is essential to ensure proper healing and to address any potential complications early on. If you have concerns about the healing process or if symptoms worsen, do not hesitate to seek medical advice. Your health and comfort are paramount, and timely intervention can help prevent more significant issues down the line.
