Administration of Leuprolide after laparoscopic surgery for chocolate cysts?
Dear Doctor, I received a dose of Leuprolide on December 24, which was the day after my laparoscopic surgery to remove chocolate cysts from both ovaries (the right one had ruptured, while the left one was intact).
Additionally, a uterine fibroid was found and removed from the left side.
It has been about two weeks post-surgery, and I received the Leuprolide injection approximately 12 days ago.
In the last couple of days, I've noticed a slight heaviness in my abdomen, similar to the feeling before menstruation.
After using the restroom, I noticed a small amount of brown discharge on the toilet paper, and sometimes there is a pinkish discharge that resembles what I experience before my period.
Is this a symptom that can occur after receiving Leuprolide? Thank you, Doctor.

Hello, Xiao Xiu: The initial effect of using leuprolide is a temporary stimulation of ovarian function.
However, its long-term effect will shift to suppressing ovarian function, which reduces the activity of the endometrium and lowers the likelihood of recurrence of endometriomas.

After undergoing laparoscopic surgery for the removal of chocolate cysts and a uterine fibroid, it is not uncommon to experience a variety of symptoms, especially after receiving a Lupron (leuprolide) injection. Lupron is a GnRH agonist that is often used to manage conditions like endometriosis and fibroids by suppressing estrogen production. This can lead to a range of side effects that may mimic premenstrual symptoms.
Given your situation, the symptoms you are experiencing—such as abdominal discomfort, a feeling of heaviness, and the presence of brownish or pinkish discharge—can indeed be associated with the administration of Lupron. Here’s a breakdown of what you might be experiencing:
1. Hormonal Changes: Lupron works by reducing estrogen levels in the body. This can lead to symptoms similar to those experienced during menstruation, including abdominal cramping or discomfort. The "heavy" feeling in your abdomen could be a result of these hormonal fluctuations.
2. Discharge: The brownish discharge you are observing is often old blood that is being expelled from the uterus. This can happen after surgery, especially if there was any bleeding during the procedure. The pinkish discharge could also be related to hormonal changes or residual effects from the surgery.
3. Post-Surgical Symptoms: After any surgical procedure, it is normal to experience some discomfort as your body heals. This can include cramping, bloating, and changes in bowel habits. The presence of discharge can also be a normal part of the healing process, especially after gynecological surgeries.
4. Timing of Symptoms: Since you mentioned that these symptoms began about 12 days after the Lupron injection, it is plausible that they are related to the hormonal changes induced by the medication. The timing aligns with the typical onset of side effects from Lupron, which can occur as the body adjusts to the decreased estrogen levels.
5. Monitoring Symptoms: It is essential to monitor your symptoms closely. If the abdominal discomfort worsens, or if you experience any additional symptoms such as fever, severe pain, or heavy bleeding, it is crucial to contact your healthcare provider immediately. These could be signs of complications such as infection or other post-operative issues.
6. Follow-Up Care: Given that you are two weeks post-surgery, it is advisable to have a follow-up appointment with your gynecologist. They can assess your symptoms in the context of your recent surgery and the Lupron injection. They may also perform an examination or imaging studies if necessary to ensure that everything is healing appropriately.
In conclusion, while the symptoms you are experiencing can be consistent with the effects of Lupron and the post-operative healing process, it is always best to consult with your healthcare provider for personalized advice and to rule out any potential complications. Your well-being is paramount, and they can provide the most accurate guidance based on your specific medical history and current condition.
