Menstrual irregularities
Hello, doctor.
In August, I had a pelvic ultrasound (since I am a virgin, I did not have a transvaginal ultrasound), and the doctor said my uterus is very healthy and the endometrium is normal.
I mentioned that I have experienced intermenstrual bleeding, which was attributed to hormonal imbalance.
Since the ultrasound did not indicate any issues with the uterus, no medication was prescribed.
However, in the past two months, I have had intermenstrual bleeding twice, occurring about a week to a week and a half after my period, with both brown and red discharge.
The shorter episodes lasted 3-4 days, while the longer ones lasted over a week.
I checked my phone records, and these episodes occurred during my ovulation period, which has made me somewhat anxious.
I had my period from December 17 to 23, and then I got my period again on January 6, which was 10 days early.
I received my first dose of the HPV vaccine on December 27.
Should I return for a follow-up appointment, or could this be related to the vaccine? Thank you, doctor.

More Info
Irregular menstrual cycles can be concerning, and it's important to understand the potential causes and when to seek medical help. Based on your description, it seems you have been experiencing some irregularities in your menstrual cycle, including mid-cycle bleeding and changes in the timing of your periods. Let's break down the possible causes and when you should consider returning to your healthcare provider.
Possible Causes of Irregular Menstrual Cycles
1. Hormonal Imbalance: As your doctor mentioned, hormonal imbalances are a common cause of irregular menstrual cycles. This can occur due to various factors, including stress, significant weight changes, excessive exercise, or underlying medical conditions such as polycystic ovary syndrome (PCOS).
2. Ovulation: Mid-cycle bleeding, also known as ovulation spotting, can occur when an egg is released from the ovary. This is typically light and lasts for a short duration. However, if the bleeding is heavier or accompanied by pain, it may warrant further investigation.
3. Infections: Pelvic inflammatory disease (PID) or other infections can cause irregular bleeding. If you experience additional symptoms such as unusual discharge, fever, or pelvic pain, it’s important to seek medical attention.
4. Uterine Conditions: While your ultrasound showed a healthy uterus, conditions like fibroids or polyps can sometimes develop and cause irregular bleeding. If you continue to experience abnormal bleeding, a follow-up ultrasound or further evaluation may be necessary.
5. Medications and Vaccines: Certain medications, including hormonal contraceptives, can affect your menstrual cycle. The HPV vaccine you received is not typically associated with changes in menstrual cycles, but individual reactions can vary. If you suspect the vaccine may be influencing your cycle, it's worth discussing with your healthcare provider.
When to Seek Help
Given your situation, here are some guidelines on when to return to your doctor:
- Persistent Irregularities: If your menstrual cycle continues to be irregular, with significant changes in timing or flow, it’s advisable to schedule a follow-up appointment.
- Accompanying Symptoms: If you experience additional symptoms such as severe pain, heavy bleeding (soaking through a pad or tampon in an hour), fever, or unusual discharge, seek medical attention promptly.
- Duration of Symptoms: If the irregular bleeding persists for more than a few cycles or if you have concerns about your menstrual health, it’s important to consult your healthcare provider for further evaluation.
- Emotional or Physical Distress: If the changes in your menstrual cycle are causing you significant anxiety or affecting your quality of life, don’t hesitate to reach out to your doctor.
Conclusion
In summary, while hormonal imbalances are a common cause of irregular menstrual cycles, it’s essential to monitor your symptoms closely. Given that you have experienced mid-cycle bleeding and changes in your menstrual timing, it would be prudent to follow up with your healthcare provider, especially if these irregularities persist or worsen. Keeping a detailed record of your menstrual cycle, including the timing and nature of any bleeding, can be helpful for your doctor in diagnosing and managing your condition effectively. Remember, your health and peace of mind are paramount, so don’t hesitate to seek help when needed.
