White spots on the inner side of the foreskin?
Hello Doctor, I have had a long-standing issue.
Every time I pull back my foreskin while showering, I notice many small white bumps clustered about one centimeter below the coronal sulcus, almost forming a complete circle.
I would like to know what this problem might be.
I have had sexual experiences but have always taken protective measures.
This condition has been present for one to two years.
Thank you, Doctor.

Hello: It is advisable to seek medical attention.
If proper precautions are taken, it is mostly a sebaceous gland issue.
Wishing you good health.

Understanding white bumps on the inner foreskin can be a common concern for many men, and it’s important to approach this topic with both sensitivity and medical knowledge. The presence of small white bumps or spots on the inner foreskin, particularly around the coronal sulcus (the groove where the head of the penis meets the shaft), can be attributed to several benign conditions.
One of the most common causes of these white bumps is Pearly Penile Papules. These are small, dome-shaped, white or flesh-colored bumps that typically appear in rows around the corona of the glans penis. They are completely normal anatomical variations and are not associated with any disease or health risk. Pearly penile papules are often mistaken for sexually transmitted infections (STIs) or other pathological conditions, but they are harmless and do not require treatment unless they cause psychological distress.
Another possibility is Fordyce Spots, which are small, painless, pale bumps that can appear on the shaft of the penis, scrotum, and inner foreskin. These spots are sebaceous glands that are not associated with hair follicles and are also considered a normal anatomical variant. Like pearly penile papules, Fordyce spots are benign and do not require treatment.
In some cases, the white bumps could be related to friction or irritation, especially if there is a history of vigorous sexual activity or if the area is not kept clean. This can lead to the development of small cysts or irritation that may appear as white bumps. Maintaining proper hygiene and ensuring that the area is clean and dry can help alleviate any irritation.
However, it is crucial to differentiate these benign conditions from more serious issues. If the white bumps are accompanied by other symptoms such as pain, itching, discharge, or changes in the skin texture, it may indicate a more serious condition such as a sexually transmitted infection (like genital warts caused by HPV) or a fungal infection. In such cases, it is essential to seek medical evaluation.
Given that you have had these bumps for one to two years without any accompanying symptoms, it is likely that they are benign. However, if you have any concerns or if there are changes in the appearance of these bumps, it is advisable to consult a healthcare professional for a thorough examination. A healthcare provider can perform a physical examination and, if necessary, conduct tests to rule out any infections or other conditions.
In summary, while white bumps on the inner foreskin can often be benign and related to normal anatomical variations, it is always best to err on the side of caution. Regular self-examinations and awareness of any changes in your body are important. If you have any doubts or experience any new symptoms, do not hesitate to reach out to a healthcare provider for personalized advice and treatment options.
