Pressing on the cheek near the left nasal bridge feels sore and soft?

Hello Doctor, about a year ago during my root canal treatment, the doctor informed me that there was a cyst issue above my left molar.
They reinforced and re-performed the root canal treatment to prevent bacteria from causing the cyst problem.
After monitoring for 8 to 9 months, it seems that the cyst has not decreased in size.
My next appointment is in March.
However, I recently noticed that when I press on my cheek near the left nasal bridge, there is a soft, tender sensation, which I didn't experience before.
Does this indicate that the cyst may have changed, and should I see a doctor promptly? What could be the possible causes of the cyst? Thank you for your assistance in confirming this.

Hello Ms.
Vivian: 1.
A cyst can be considered as an extension of a dental root canal infection that spreads to the bone surrounding the apex of the tooth, resulting in bone destruction and the formation of a cyst.
2.
If you feel discomfort when pressing on the area near the left side of your nose, it is advisable to consult a dentist for further diagnosis and treatment.

Facial pain, especially around the nose and cheek area, can be concerning, particularly when there is a known cyst involved. Based on your description, it seems that you have a history of a cyst associated with your left upper molar, which was monitored after root canal treatment. The recent development of tenderness or a "soft" feeling when pressing on the area near your left nasal bridge could indicate a change in the cyst or surrounding tissues.

Cysts in the dental region, particularly those associated with teeth, can arise from various causes, including chronic inflammation, infection, or even developmental anomalies. In your case, the cyst may have formed as a result of the infection that led to the need for root canal therapy. If the cyst has not changed in size over the past several months, it might have been stable; however, any new symptoms, such as pain or tenderness, warrant further investigation.

The presence of pain or discomfort in the area could suggest several possibilities:
1. Cyst Growth or Infection: If the cyst has become infected or is growing, it could lead to increased pressure and discomfort in the surrounding tissues. This could also explain the new symptoms you are experiencing.

2. Sinus Involvement: Given the proximity of the upper teeth to the maxillary sinus, any issues with the cyst could potentially affect the sinus, leading to sinusitis or other sinus-related symptoms, which can also cause facial pain.

3. Nerve Involvement: The area you describe is innervated by branches of the trigeminal nerve. If the cyst is pressing on or irritating these nerves, it could lead to pain or discomfort in the facial region.

4. Muscle Tension or Other Dental Issues: Sometimes, pain in the facial area can be attributed to muscle tension or other dental issues, such as bruxism (teeth grinding), which can lead to referred pain in the face.

Given these considerations, it is advisable to follow up with your dentist or an oral surgeon as soon as possible. They may recommend imaging studies, such as a dental X-ray or a CT scan, to assess the cyst's current status and determine if any intervention is necessary.
In terms of what could have caused the cyst, as mentioned earlier, it is often related to chronic inflammation or infection stemming from dental issues. Other factors that could contribute include:
- Poor Oral Hygiene: This can lead to infections that may cause cyst formation.

- Trauma: Any injury to the area could potentially lead to cyst development.

- Genetic Factors: Some individuals may be more prone to developing cysts due to genetic predispositions.

In conclusion, while the cyst may have been stable for a time, the new symptoms you are experiencing suggest that it is prudent to seek further evaluation. Early intervention can prevent potential complications and ensure that any necessary treatment is administered promptly. Always prioritize your health and do not hesitate to reach out to your healthcare provider with any concerns.
