Consult a doctor?
Hello Doctor, I tested positive for COVID-19 a few days ago and have a few questions.
I am currently taking Qingguo No.
1 and have also visited an ENT specialist for medication to treat my sore throat and runny nose.
I noticed that my phlegm has been yellow and thick, sometimes with blood (occasionally just streaks of blood, but other times it looks quite alarming).
Is this normal? The ENT doctor said it’s not a concern since I didn’t experience this during my previous COVID-19 diagnosis about two years ago.
Additionally, I occasionally feel pain at the back of my head when blowing my nose, and the doctor mentioned it might be due to blowing too hard, so I shouldn’t worry about issues like blood vessel rupture.
Lastly, while visiting my grandmother's house, I noticed four red, itchy, and slightly swollen spots on my left wrist.
I applied insect repellent, which helped a bit, but they still itch.
Today, I found a small bump on the top of my foot.
I looked online and saw that red rashes can be associated with HIV...
I'm concerned if my immune system is weakened due to the COVID-19 diagnosis and if I could get infected in a public restroom.
However, I'm not sure if the rash I have is related to that.
My family thinks it’s likely insect bites.
Since my anxiety has increased after the diagnosis, I hope you can provide some insight.
Thank you!

1.
After several days of forceful coughing, it is normal to have some blood-tinged sputum due to the rupture of small blood vessels in the trachea.
However, if there is a significant amount of fresh blood, it is different, and you should seek medical attention.
2.
Whether it's from an insect bite or a skin issue caused by contact with dirty objects, you can first wash the area with clean water, then apply an ointment or cover it with a bandage.
If the symptoms worsen, you should see a doctor.

It's understandable to feel anxious and concerned about your health, especially after testing positive for COVID-19. Let's address your questions and concerns one by one.
1. Yellow and Blood-Streaked Phlegm: The presence of yellow, thick phlegm can indicate that your body is fighting an infection, which is common during respiratory illnesses like COVID-19. The yellow color typically suggests that your immune system is responding to an infection, as white blood cells accumulate in the mucus. However, the presence of blood in your phlegm, even if it’s just streaks, can be concerning. While it can sometimes occur due to irritation in the throat or respiratory tract, it’s essential to monitor this symptom closely. If the blood persists or increases in quantity, or if you experience additional symptoms such as chest pain or difficulty breathing, you should seek medical attention promptly.
2. Head Pain When Blowing Your Nose: It’s not uncommon to experience discomfort or pain in the head when blowing your nose, especially if you’re doing it forcefully. This can be due to increased pressure in the sinuses or even minor trauma to the blood vessels in the nasal passages. However, if the pain is severe, persistent, or accompanied by other symptoms like fever or swelling, it would be wise to consult with a healthcare provider to rule out any sinus infection or other complications.
3. Red, Itchy Bumps on Your Wrist: The red, itchy bumps on your wrist could very well be insect bites, especially if you’ve been in an area where bugs are prevalent. Applying anti-itch cream was a good first step. However, if these bumps worsen, spread, or are accompanied by other symptoms like fever or swelling, it would be prudent to have them evaluated by a healthcare professional.
4. Concerns About HIV and Immunity: Given your history of being HIV positive, it’s natural to worry about your immune system's ability to handle infections. However, if you are on effective antiretroviral therapy and your viral load is undetectable, your immune system is likely functioning well. It’s important to maintain your medication regimen and have regular check-ups with your healthcare provider to monitor your CD4 counts and overall health.
5. Anxiety and Health Concerns: Your heightened anxiety after a COVID-19 diagnosis is completely understandable. The pandemic has caused a lot of stress for many individuals, and it’s important to address these feelings. Consider reaching out to a mental health professional if your anxiety becomes overwhelming. They can provide coping strategies and support to help you manage your fears.
6. Infection Risks: Regarding your concern about contracting infections in public restrooms, while it’s always wise to practice good hygiene (like washing hands thoroughly), the risk of contracting HIV or other infections in such settings is generally low, especially if you are not engaging in high-risk behaviors.
In summary, while some of your symptoms may be typical of a respiratory infection, the presence of blood in your phlegm and the new skin reactions should be monitored closely. Don’t hesitate to reach out to your healthcare provider for further evaluation and reassurance. Remember, taking care of your mental health is just as important as addressing physical symptoms, especially during these challenging times.
