Post-circumcision care?

Hello doctor, I had a circumcision two years ago, and the swelling still hasn't gone down.
Sometimes it reduces a bit, but then it swells up again after I wake up.
I've had follow-up visits two or three times, and the doctors have said it will eventually resolve, prescribing some ointment, but I'm still very concerned since it's been a long time.
I would like to ask what the situation might be and how I should address it.

Hello: Based on personal experience, if there is swelling, it is likely not serious.
Most of the time, using scar ointment for a period will help.
Wishing you good health.

More Info

Persistent swelling after circumcision can be concerning, especially if it has lasted for an extended period, such as two years in your case. While some swelling can be a normal part of the healing process, especially in the weeks and months following the procedure, prolonged swelling may indicate underlying issues that need to be addressed.

Possible Causes of Persistent Swelling
1. Infection: One of the most common causes of persistent swelling is an infection at the surgical site. Signs of infection may include redness, warmth, discharge, or increased pain. If you notice any of these symptoms, it’s essential to consult your healthcare provider.

2. Scar Tissue Formation: After any surgical procedure, scar tissue can form. In some cases, excessive scar tissue can lead to swelling and discomfort. This is known as hypertrophic scarring or keloid formation, which can sometimes require treatment.

3. Lymphatic Issues: The lymphatic system plays a crucial role in fluid drainage from tissues. If there is an issue with lymphatic drainage in the area, it can lead to persistent swelling. This condition is known as lymphedema.

4. Allergic Reaction: If you have been using any topical medications or ointments, there is a possibility of an allergic reaction causing localized swelling. It’s important to review any products you have been using with your doctor.

5. Underlying Medical Conditions: Conditions such as diabetes or vascular issues can affect healing and fluid retention, potentially leading to prolonged swelling.

6. Surgical Technique: Sometimes, the technique used during circumcision can lead to complications. If too much skin was removed or if the remaining skin is too tight, it may cause swelling and discomfort.

Solutions and Recommendations
1. Follow-Up with a Specialist: Since you have already seen your doctor multiple times, it may be beneficial to seek a second opinion from a urologist or a specialist in male reproductive health. They can provide a more thorough evaluation and possibly suggest further diagnostic tests.

2. Imaging Studies: If there is suspicion of underlying issues, imaging studies such as an ultrasound may be recommended to assess the tissue and lymphatic drainage in the area.

3. Topical Treatments: If your doctor has prescribed a topical ointment, ensure you are using it as directed. If you suspect an allergic reaction, discuss alternative treatments with your healthcare provider.

4. Compression Garments: In some cases, wearing a supportive garment may help reduce swelling by providing gentle compression to the area.

5. Surgical Revision: If the swelling is due to excessive scar tissue or surgical technique issues, a revision surgery may be necessary. This would involve a discussion with your surgeon about the risks and benefits.

6. Lifestyle Modifications: Maintaining a healthy lifestyle, including proper hydration, nutrition, and avoiding irritants, can support overall healing.

7. Monitor Symptoms: Keep track of any changes in swelling, pain, or other symptoms. If you notice worsening conditions, seek medical attention promptly.

Conclusion
Persistent swelling after circumcision can arise from various causes, and while some degree of swelling can be normal, prolonged symptoms warrant further investigation. It’s crucial to communicate openly with your healthcare provider about your concerns and to advocate for further evaluation if necessary. Your health and comfort are paramount, and there are solutions available to address your situation.
