Medication consultation
Due to my loss of appetite, my weight decreased from 52 kg in September 2022 to 44 kg in February 2023.
The medication I took had no effect.
On February 19, 2023, I consulted a traditional Chinese medicine doctor who prescribed the following:
- An Zhong San, 3.0 g (total 15.0 g)
- Ban Xia Xie Xin Tang, 3.0 g (total 15.0 g)
- Shan Zha, 1.0 g (total 5.0 g)
- Mai Ya, 1.0 g (total 5.0 g)
- Mu Li, 1.0 g (total 5.0 g)
- Zhe Bei Mu, 1.0 g (total 5.0 g)
- Hou Po, 1.0 g (total 5.0 g)
- Chen Pi, 0.5 g (total 2.5 g)
I was instructed to take 2 teaspoons after each meal.
On the first day, after taking the medication, I woke up to use the bathroom and could not fall back asleep.
The second day, the same situation occurred, but I fell asleep even later.
On the third day, I was unable to sleep the entire night, feeling very alert with no sleepiness at all.
I would like to ask if the insomnia I am experiencing is due to the medication I am taking, or if it is a personal issue.
Thank you.

Dr. Peng Renyuan reply Traditional Chinese Medicine
Hello Ms.
You: Based on your prescription, it seems unrelated to sleep issues.
As for the appetite loss and weight loss you mentioned, you may want to discuss with your primary physician whether there are emotional factors influencing these symptoms, and then determine a treatment plan.
Wishing you good health and peace of mind!

The impact of Traditional Chinese Medicine (TCM) on sleep and appetite can be quite significant, especially for individuals experiencing issues like insomnia and decreased appetite. In your case, it seems that the herbal prescriptions you received, specifically An Zhong San and Ban Xia Xie Xin Tang, may have contributed to your sleep disturbances.
Firstly, it’s important to understand that TCM operates on the principle of balancing the body's energies (Qi) and addressing the root causes of ailments rather than just the symptoms. The herbs prescribed to you are known for their various effects on the body. For instance, An Zhong San is often used to calm the mind and regulate the digestive system, while Ban Xia Xie Xin Tang is traditionally used to harmonize the stomach and alleviate symptoms of nausea and discomfort. However, the combination of these herbs can sometimes lead to unexpected side effects, particularly if they are not well-suited to your individual constitution or if there are underlying conditions that have not been addressed.
From your description, it appears that the herbal treatment may have stimulated your central nervous system, leading to heightened alertness and difficulty sleeping. This is not uncommon with certain herbal formulations, especially those that include ingredients known for their stimulating properties. For example, herbs like Chen Pi (tangerine peel) and Hou Po (magnolia bark) can have a warming effect and may stimulate digestion, which could inadvertently affect your ability to relax and fall asleep.
In TCM, sleep disturbances can often be linked to imbalances in the heart and kidney systems, or issues with the spleen and stomach. If the herbs you were prescribed are not addressing the root cause of your insomnia, or if they are causing an imbalance, this could explain your experience of increased alertness and difficulty sleeping. It’s crucial to communicate these side effects to your TCM practitioner, as they may need to adjust your treatment plan or consider alternative herbs that are more calming and supportive for your specific needs.
Additionally, the relationship between appetite and sleep is well-documented in both TCM and Western medicine. Poor sleep can lead to decreased appetite and weight loss, while a lack of proper nutrition can further exacerbate sleep issues. The body requires adequate nutrients to function optimally, including for the production of neurotransmitters that regulate sleep. If your appetite remains suppressed, it could create a cycle of insomnia and nutritional deficiency that is difficult to break.
In terms of managing your situation, it may be beneficial to explore other TCM modalities such as acupuncture, which can help to regulate the body's energy and promote relaxation. Acupuncture has been shown to improve sleep quality and can be a complementary approach alongside herbal medicine. Furthermore, lifestyle modifications such as establishing a calming bedtime routine, practicing mindfulness or meditation, and ensuring a conducive sleep environment can also support better sleep.
If your symptoms persist despite adjustments in your TCM treatment, it may be worthwhile to consult with a Western medical professional. They can provide additional insights and possibly recommend further evaluations to rule out any underlying medical conditions that could be contributing to your symptoms.
In summary, while TCM can be effective for managing sleep and appetite issues, it is essential to tailor treatments to the individual. Open communication with your practitioner about your experiences and side effects is key to finding a suitable approach that addresses both your sleep disturbances and appetite concerns.
