Which specialty should I see?

Hello, this morning when I woke up, my vision was foggy and white.
After walking a few steps, I decided to sit down, but when I sat down, my vision turned completely white and I couldn't see anything at all.
The sounds I heard also became muffled, lasting for almost a minute.
Later, when I squatted down and tried to feel my way out to seek help from my family, my vision gradually returned, and my hearing also slowly improved.
Which specialist should I see? Thank you!

Dr. Cai Dongqi reply Family Medicine

Hello, Xiao Jing: First, visit a neurologist to check if there is a temporary insufficient blood supply to the brain.
If not, then proceed to see an ophthalmologist.

Based on your description of sudden vision and hearing issues, it is crucial to seek immediate medical attention. The symptoms you experienced—sudden blurriness followed by complete loss of vision and muffled hearing—could indicate a serious underlying condition that requires prompt evaluation by a healthcare professional.

Given the nature of your symptoms, you should consider seeing a specialist in both ophthalmology and otolaryngology (ear, nose, and throat specialist, often referred to as ENT). Here’s why both specialists are relevant:
1. Ophthalmologist: Since your primary concern involves sudden changes in vision, an ophthalmologist is essential for assessing your eye health. Sudden vision loss can be caused by various conditions, including retinal detachment, vitreous hemorrhage, or acute glaucoma. An ophthalmologist can perform a comprehensive eye examination, including visual acuity tests, a dilated eye exam, and possibly imaging studies to determine the cause of your symptoms.

2. Otolaryngologist (ENT): The muffled hearing you experienced alongside your vision issues suggests that there may be an issue with your auditory system. An ENT specialist can evaluate your hearing and assess for conditions such as ear infections, Eustachian tube dysfunction, or other auditory pathologies that could contribute to your symptoms. They may perform hearing tests and examine your ear canals and eardrums to identify any abnormalities.

In addition to these specialists, if you experience any other concerning symptoms such as dizziness, balance issues, or neurological symptoms (like weakness or numbness), it may also be advisable to consult a neurologist. Neurological conditions can sometimes present with both visual and auditory disturbances, and a neurologist can help rule out any central nervous system issues.

Additional Considerations:
- Emergency Care: If your symptoms recur or worsen, consider going to an emergency room. Sudden vision and hearing loss can sometimes indicate a stroke or transient ischemic attack (TIA), which requires immediate intervention.

- Follow-Up: After your initial evaluations, follow-up appointments with the appropriate specialists will be necessary to monitor your condition and adjust any treatments as needed.

- Lifestyle Factors: While awaiting your appointments, ensure you stay hydrated and avoid any activities that could put you at risk of falls or further injury, especially if you experience dizziness or balance issues.

- Documentation: Keep a record of your symptoms, including when they occur, their duration, and any other accompanying symptoms. This information can be invaluable for your healthcare providers in diagnosing your condition.

In summary, seeing both an ophthalmologist and an ENT specialist is crucial for a comprehensive evaluation of your sudden vision and hearing issues. Prompt medical attention can help identify the underlying cause and initiate appropriate treatment, ensuring the best possible outcome for your health.
