Regarding the impact of antidepressant dosage and antipsychotic medications on decreased libido?
Hello Doctor, I would like to ask if the side effects of low libido and difficulty achieving orgasm caused by taking antidepressants and antipsychotic medications will resolve if I stop taking them.
I have been on a high dose of Sertraline and 150 mg of Quetiapine for a long time.
I do not intend to stop taking them; I am just concerned whether these side effects are permanent and irreversible, or if they will go away if I stop taking the medications.
Thank you, Doctor.
P.S.
I have been on both of these medications for a long time.

Dr. Cai Yunlin reply Psychiatry
Hello, in general, sexual dysfunction related to antidepressants and antipsychotic medications is usually reversible.
However, there are several factors (such as physiological diseases, mental disorders, substance abuse, and other prescribed medications) that may be associated with sexual dysfunction.
It is recommended that you discuss your concerns further with your physician for appropriate assistance.
Thank you.

The concerns you have regarding the sexual side effects of antidepressants and antipsychotics are quite common and deserve careful consideration. Many individuals who are prescribed these medications report experiencing changes in their sexual function, including decreased libido, difficulty achieving orgasm, and other related issues.
Antidepressants, particularly selective serotonin reuptake inhibitors (SSRIs) like Paroxetine (which you mentioned), are known to cause sexual side effects in a significant number of patients. These effects can include reduced sexual desire, delayed orgasm, and erectile dysfunction in men. The mechanism behind this is believed to be related to the way these medications increase serotonin levels in the brain, which can inhibit sexual arousal and response.
Similarly, antipsychotic medications, such as the one you are taking (likely referring to Olanzapine or a similar drug), can also contribute to sexual dysfunction. These medications can affect dopamine pathways in the brain, which play a crucial role in sexual desire and arousal. The side effects can vary depending on the specific medication and the individual’s response to it.
Regarding your question about whether these side effects are permanent or reversible, the good news is that in many cases, they are reversible. Once the medication is discontinued or adjusted, many patients find that their sexual function returns to baseline levels. However, this is not universally true for everyone, and some individuals may experience lingering effects even after stopping the medication. Factors such as the duration of medication use, the specific drugs involved, and individual physiology can all play a role in determining whether the side effects persist.
It's also important to consider that sexual dysfunction can be influenced by a variety of factors beyond medication, including psychological aspects such as depression itself, relationship dynamics, and overall health. Therefore, it’s crucial to have an open dialogue with your prescribing physician about your concerns. They may suggest alternative medications that have a lower risk of sexual side effects, or they might recommend strategies to mitigate these effects, such as adjusting the dosage or adding another medication to counteract the side effects.
In summary, while the sexual side effects associated with antidepressants and antipsychotics can be distressing, they are often reversible. It is essential to communicate with your healthcare provider about your experiences and concerns. They can help you navigate these issues and explore potential solutions to improve your sexual health while managing your mental health conditions effectively. Remember, you are not alone in this, and there are options available to help you maintain both your mental and sexual well-being.
