Interactions of contraceptive pills?

Hello, I have been taking Yaz for about a year without interruption.
On March 3, I took the first pill of this cycle, and from March 9 to March 12, I applied a topical tetracycline ointment to my skin wound once daily.
On March 13, I had unprotected intercourse, and on March 14, I found out that tetracycline could potentially reduce the effectiveness of birth control.
I took Ella (30 hours after intercourse) and would like to ask the following questions: 1.
What is the likelihood of pregnancy in this situation? 2.
Can I continue taking Yaz as scheduled daily (which means I would be taking both the emergency contraceptive and the regular birth control pill today)? 3.
If I continue taking Yaz this cycle, will it affect the efficacy of the emergency contraceptive? Thank you for taking the time to answer my questions.

Ajd, 30~39 year old female. Ask Date: 2024/03/14

When it comes to understanding the interaction between birth control pills and tetracycline, it's essential to consider how these medications work and their potential effects on each other.
1. Potential for Pregnancy: You mentioned that you have been taking birth control pills (Yaz) consistently for about a year and that you applied a topical tetracycline ointment from March 9 to March 12. Generally, tetracycline, when used topically, does not significantly affect the efficacy of oral contraceptives. However, if you were to take tetracycline orally, there could be a risk of reduced effectiveness of the birth control pill. Since you applied the ointment and did not take oral tetracycline, the risk of pregnancy remains low, provided you have been taking your birth control pills correctly. However, since you had unprotected intercourse on March 13, and considering the timing of your birth control pill intake, there is still a slight chance of pregnancy, especially if there was any missed dose or if the pill was not taken consistently.

2. Continuing Birth Control Pills: You can continue taking your birth control pills as scheduled. It is crucial to maintain the regularity of your pill intake to ensure maximum effectiveness. If you have been taking Yaz consistently, you should continue to do so, even if you are using emergency contraception (Ella) after unprotected intercourse. The emergency contraceptive is designed to prevent ovulation and is most effective when taken as soon as possible after unprotected sex.

3. Interaction with Emergency Contraceptive: Continuing to take your regular birth control pills while also taking Ella should not negatively affect the efficacy of the emergency contraceptive. However, it is important to note that Ella can delay ovulation, and if you are already on birth control, it may not be necessary to take Ella unless you are concerned about potential contraceptive failure. After taking Ella, you should continue with your regular birth control regimen as usual.
In summary, while tetracycline can potentially interact with oral contraceptives, the topical application you used should not have affected your birth control's effectiveness. You can continue taking Yaz as scheduled, and taking Ella should not interfere with your ongoing birth control regimen. However, if you have any concerns about potential pregnancy or the effectiveness of your contraceptive methods, it is always best to consult with your healthcare provider for personalized advice and guidance.
Additionally, it's important to remember that while emergency contraceptives are effective, they should not be used as a regular form of birth control. Consistent use of your birth control pills is the best way to prevent unintended pregnancy. If you have any further questions or concerns, don't hesitate to reach out to your healthcare provider for clarification and support.
