Herpes issues?
Hello, doctor.
One morning, I woke up and felt a bit of pain while urinating.
Subsequently, I noticed some red spots developing on my genital area.
It has been about a week now, and there are small sores on both the glans and the foreskin (similar to mouth ulcers, but much smaller).
There are especially many on the foreskin, and they are itchy and slightly swollen.
The doctor thinks it might be herpes.
However, I researched genital herpes and haven't seen any blisters in the past week, and these small sores are not very concentrated.
What could these small sores indicate? I have been quite distressed for the past two to three days.
Thank you, doctor.

Hello: Blisters should occur before wounds, so it should have already happened.
Wishing you good health.

More Info
Genital sores can be concerning and uncomfortable, and it's understandable that you're feeling anxious about your symptoms. Based on your description, it sounds like you are experiencing small sores on the glans and foreskin of your penis, accompanied by itching and swelling. While I cannot provide a definitive diagnosis without a physical examination, I can offer some insights into possible causes and concerns regarding your symptoms.
1. Herpes Simplex Virus (HSV): Although you mentioned not seeing any blisters, genital herpes can sometimes present with small sores or ulcers that may not develop into the classic vesicular (blister) stage. The initial outbreak can be accompanied by pain, itching, and swelling, and sores may appear in various locations. It's important to note that herpes can be asymptomatic or present with atypical symptoms.
2. Balanitis: This is an inflammation of the glans (the head of the penis) and can result from various causes, including infections (bacterial or fungal), irritants (such as soaps or lotions), or underlying skin conditions. Symptoms often include redness, swelling, itching, and sometimes sores or lesions.
3. Contact Dermatitis: If you've recently changed soaps, detergents, or personal care products, you might be experiencing an allergic reaction or irritation, leading to redness, itching, and sores.
4. Candidiasis (Yeast Infection): While more common in women, men can also develop yeast infections, particularly if they are uncircumcised. Symptoms may include itching, redness, and the development of small sores.
5. Sexually Transmitted Infections (STIs): Other STIs, such as syphilis or chancroid, can also cause sores in the genital area. These infections often require specific testing and treatment.
6. Lichen Planus: This is a chronic inflammatory condition that can affect the skin and mucous membranes, including the genital area. It can cause itchy, painful lesions that may resemble sores.
Given the duration of your symptoms (about a week) and the discomfort you're experiencing, it is crucial to seek medical attention. A healthcare provider can perform a physical examination, take a detailed history, and possibly conduct laboratory tests (such as swabs of the sores or blood tests) to determine the exact cause of your symptoms.
In the meantime, here are some general recommendations:
- Avoid Irritants: Refrain from using any new soaps, lotions, or personal care products that may irritate the area.
- Maintain Hygiene: Keep the area clean and dry. Gently wash with mild soap and water, and avoid scrubbing the sores.
- Avoid Sexual Activity: Until you have a clearer understanding of your condition, it's advisable to avoid sexual activity to prevent potential transmission of infections and further irritation.
- Pain Relief: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help alleviate discomfort.
It's understandable to feel distressed about these symptoms, but remember that many conditions can be effectively treated once properly diagnosed. Please make an appointment with a healthcare provider as soon as possible to discuss your symptoms and receive appropriate care. Your health and peace of mind are important, and addressing these concerns promptly is the best course of action.
