Delayed menstruation?

Hello Dr.
Chang, my last menstrual period started on February 22 and ended on February 28.
In March (from February 22 to March 31), I had sexual intercourse 1 or 2 times without a condom for 1 to 2 minutes, but I used a condom for the rest of the time.
However, my period still hasn't arrived today, April 2.
What should I do? Am I pregnant, and should I see a doctor?

Xiao Li, 20~29 year old female. Ask Date: 2024/04/02

Hello, the questioner: There is still a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
It is recommended to check for pregnancy if your period is more than a week late.
Thank you! Wishing you peace and health.

If you have missed your period and are concerned about the possibility of pregnancy, it is essential to consider several factors before deciding whether to see a doctor. Given your last menstrual period started on February 22 and ended on February 28, and you had unprotected intercourse on a couple of occasions in March, it is reasonable to suspect that pregnancy could be a possibility, especially since your period is now overdue.

The first step you should take is to perform a home pregnancy test. These tests are designed to detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining. Most home pregnancy tests are quite accurate when taken after a missed period, so it would be advisable to wait a few days past your expected period for the most reliable result. If the test is positive, it is a strong indication that you are pregnant, and you should schedule an appointment with your healthcare provider for further evaluation and prenatal care.

If the test is negative but your period still does not arrive, it is essential to consider other factors that could contribute to a missed period. Stress, significant weight changes, excessive exercise, hormonal imbalances, and certain medical conditions can all affect your menstrual cycle. If you have experienced any of these changes or if your periods have been irregular in the past, it may explain the delay.

In your case, since you have had unprotected intercourse, it is prudent to take a pregnancy test regardless of any other symptoms. If you experience any unusual symptoms such as severe abdominal pain, heavy bleeding, or any other concerning signs, you should seek medical attention immediately, as these could indicate complications such as an ectopic pregnancy or miscarriage.

If you continue to experience missed periods and negative pregnancy tests, it would be wise to consult with a healthcare provider. They can perform blood tests to check hCG levels and assess your overall reproductive health. Additionally, they may conduct a pelvic exam or ultrasound if necessary to rule out any underlying issues.

In summary, if your period is late and you have had unprotected sex, taking a home pregnancy test is the first step. If the test is positive, follow up with your doctor. If it is negative and your period does not come, or if you have any concerning symptoms, it is advisable to see a healthcare provider for further evaluation. Remember, early detection and care are crucial for a healthy pregnancy if you are indeed pregnant, and addressing any menstrual irregularities can help maintain your reproductive health.
