There is an additional lump above the testicle?

Hello, doctor.
Recently, I suddenly noticed a lump above my testicle (under the skin).
It is soft, oval-shaped, and slightly narrower than a regular pencil.
It seems to be connected to the testicle below it, located near the lower left side of the base of the penis.
The protrusion is visible from the outside, but I'm not sure how long it has been there.
What could be the possible causes?

If there is an unexplained lump in the testicle, it is recommended to seek medical evaluation at a urology clinic as soon as possible.

When it comes to soft lumps above the testicles, there are several potential causes that should be considered. It’s important to approach this situation with a clear understanding of the anatomy and possible medical conditions that could lead to the development of such lumps.

Firstly, one of the most common causes of soft lumps in the scrotal area is a hydrocele. A hydrocele is a fluid-filled sac surrounding a testicle that results in swelling. It is usually painless and can vary in size. Hydroceles are often benign and may not require treatment unless they cause discomfort or become very large.

Another possibility is an epididymal cyst, which is a benign, fluid-filled sac that forms in the epididymis, the coiled tube at the back of the testicle that stores and carries sperm. These cysts are typically painless and can be felt as soft lumps. They are also generally harmless and do not require treatment unless they cause pain or discomfort.

A spermatocele is another type of cyst that can occur in the epididymis. Similar to an epididymal cyst, a spermatocele is filled with fluid and may contain sperm. These cysts are also usually painless and may not require treatment.

In some cases, the lump could be a varicocele, which is an enlargement of the veins within the scrotum. Varicoceles can feel like a "bag of worms" and are often associated with discomfort or a dull ache. They can affect fertility, so if you are experiencing symptoms related to this, it may warrant further investigation.

Less commonly, the lump could be related to testicular tumors, which can be benign or malignant. Testicular cancer often presents as a lump or swelling in the testicle itself, but it can sometimes present as a lump in the surrounding area. It is crucial to monitor any changes in size, shape, or associated symptoms such as pain or discomfort.

Infections such as epididymitis or orchitis can also cause swelling and lumps in the scrotal area. These conditions are often accompanied by pain, redness, and swelling, and they require medical treatment.

Given the variety of potential causes, it is essential to seek a medical evaluation for any new lumps or changes in the scrotal area. A healthcare provider will typically perform a physical examination and may recommend imaging studies, such as an ultrasound, to determine the nature of the lump.
In summary, while soft lumps above the testicles can be caused by benign conditions such as hydroceles, cysts, or varicoceles, it is important to rule out more serious conditions, including testicular tumors. If you notice any changes, it is advisable to consult a healthcare professional for an accurate diagnosis and appropriate management. Early intervention can help address any underlying issues and provide peace of mind.
