Sudden pain below the left testicle?
Since high school, I have been increasing the frequency of masturbation, and now I am in my junior year of college.
Recently, I have experienced prolonged erections, and when I apply light pressure to the left testicle, there is some pain accompanied by a sensation similar to abdominal cramps.
Lying flat seems to alleviate the discomfort.
The next day, I usually feel fine, but after another prolonged erection, the pain returns, although it is not worse than the previous day.
This is the first time I have encountered this situation, and it seems to coincide with longer-lasting erections.
Could this be a case of varicocele? After reading about varicocele online, the symptoms seem quite similar.
I have noticed slight swelling in the blood vessels and the testicle below the left testicle, and the main discomfort feels like cramping.
Recently, I have been staying up late and have gained a significant amount of weight.
My biggest concern is varicocele, or could this be due to inflammation from possibly masturbating three to four times a day? I have also noticed that my semen has become much more watery and less viscous compared to before.
Thank you for your assistance, doctor.

More Info
Understanding left testicular pain can be complex, as it may arise from various causes, including anatomical, physiological, or even psychological factors. Based on your description, it seems you are experiencing discomfort in your left testicle, particularly during prolonged erections, accompanied by abdominal pain. You also mentioned that lying down alleviates the discomfort, and you have noticed some swelling in the area.
One potential cause of your symptoms could indeed be a condition known as varicocele, which is characterized by the enlargement of veins within the scrotum, similar to varicose veins in the legs. Varicoceles are more common on the left side due to the anatomical differences in venous drainage from the testicles. They can lead to a dull ache or discomfort, especially after prolonged periods of standing or during activities that increase abdominal pressure, such as heavy lifting or sexual arousal. The pain may also be exacerbated by prolonged erections, as increased blood flow can lead to further engorgement of the affected veins.
Another possibility is epididymitis, which is inflammation of the epididymis, often caused by infection. This condition can lead to pain and swelling in the testicle and may be associated with other symptoms such as fever, redness, or warmth in the area. Given that you mentioned an increase in sexual activity, it’s important to consider whether there might be an underlying infection, especially if you experience any discharge or changes in urination.
Your mention of abdominal pain that feels similar to gastrointestinal discomfort could also suggest that the pain is referred from another area, such as the lower abdomen or pelvis. Conditions like inguinal hernias or issues related to the gastrointestinal tract can sometimes present with testicular pain.
The changes in your semen consistency, described as being more watery than usual, could also be related to various factors, including hydration levels, frequency of ejaculation, or even hormonal changes. Frequent ejaculation can lead to temporary changes in semen quality, but if you notice persistent changes, it may warrant further investigation.
Given your concerns about varicocele and the symptoms you are experiencing, it would be advisable to consult a healthcare professional, preferably a urologist. They can perform a physical examination and possibly an ultrasound to assess the blood flow in the veins of the scrotum and rule out any significant issues.
In the meantime, consider the following recommendations:
1. Avoid prolonged periods of standing or activities that increase abdominal pressure, which may exacerbate your symptoms.
2. Wear supportive underwear to help alleviate discomfort.
3. Maintain a healthy lifestyle, including regular exercise and a balanced diet, to manage weight and overall health.
4. Limit sexual activity if it seems to trigger or worsen your symptoms until you have a clearer understanding of the cause.
In conclusion, while varicocele is a plausible explanation for your symptoms, other conditions could also be at play. A thorough evaluation by a healthcare provider is essential to determine the exact cause of your left testicular pain and to develop an appropriate treatment plan.
