Postcoital bleeding

Doctor, my last menstrual period ended around April 4th.
I had sexual intercourse on April 5th, and about six hours later, I started noticing brown discharge.
By April 10th, there was suddenly a large amount of brown discharge mixed with blood, and there was also a faint pink color when I wiped.
Starting April 13th, I began to see bright red blood along with the brown discharge.
Since it has been over a week and on April 14th, I noticed bright red blood when I wiped, but I have no abdominal discomfort.
What should I do, or is there something wrong?

CC, 10~19 year old female. Ask Date: 2024/04/14

Hello, the factors affecting intermenstrual bleeding are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, postcoital contraception, diet, polycystic ovary syndrome, lesions of the uterus or cervix, infections, ovulation, pregnancy, and breastfeeding, among others.
It is advisable to consult a gynecologist if the symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Chang.

Post-intercourse bleeding can be concerning, and it’s important to understand the potential causes and when to seek medical attention. Based on your description, it seems that you experienced a range of bleeding symptoms following sexual intercourse, including brown discharge, pink spotting, and eventually bright red blood. Here are some possible explanations for your symptoms:
1. Ovulation Spotting: If your menstrual cycle is regular, the timing of your sexual activity could coincide with ovulation, which can sometimes cause light spotting due to hormonal changes. This is usually not a cause for concern.

2. Cervical Irritation: Sexual intercourse can sometimes irritate the cervix, especially if there is any underlying condition such as cervicitis (inflammation of the cervix) or cervical polyps. This irritation can lead to light bleeding or spotting.

3. Hormonal Fluctuations: Changes in hormone levels can lead to irregular bleeding. If you have recently changed birth control methods or are experiencing stress, this could also contribute to your symptoms.

4. Infections: Sexually transmitted infections (STIs) or other infections of the reproductive tract can cause bleeding. If you have any unusual discharge, odor, or discomfort, it’s important to get tested.

5. Pregnancy-related Issues: If there is a possibility of pregnancy, bleeding can occur due to implantation bleeding or other complications such as a miscarriage or ectopic pregnancy. If you suspect you might be pregnant, consider taking a pregnancy test.

6. Endometrial Issues: Conditions such as endometriosis or fibroids can cause abnormal bleeding. If you have a history of these conditions, they may be exacerbated by sexual activity.

Given that your symptoms have persisted for over a week and have evolved from brown discharge to bright red blood, it is advisable to seek medical attention. Here are some signs that warrant immediate consultation with a healthcare provider:
- Heavy Bleeding: If you are soaking through pads or tampons in an hour or less.

- Severe Pain: If you experience significant abdominal or pelvic pain.

- Fever or Chills: These could indicate an infection.

- Unusual Discharge: If you notice a foul odor or unusual color in your discharge.

When you visit your healthcare provider, they may perform a pelvic exam, conduct tests for STIs, and possibly order an ultrasound or other imaging studies to assess your reproductive organs. It’s important to communicate all your symptoms clearly, including the timing of your menstrual cycle and any other changes you’ve noticed.

In summary, while some causes of post-intercourse bleeding can be benign, the persistence and change in the nature of your bleeding suggest that a medical evaluation is warranted. Early intervention can help address any underlying issues and provide peace of mind.
