Anal issues, anal itching?
Recently, I've been experiencing alternating constipation and diarrhea.
After using the restroom, I feel like I haven't completely emptied my bowels, and every time I wipe, I end up with skin irritation.
I also have itching around the anal area.
I would like to ask the doctor what these symptoms might indicate.

Dr. Zhang Shuhao reply Surgery
The situation you described is known as pruritus ani.
The skin in the perianal area is very sensitive.
Excessive wiping or cleaning with hot water and bath products can irritate the skin, stripping away too much sebum and causing itching.
It is recommended to avoid excessive cleaning, such as rinsing with cool water after using the restroom and gently patting dry; wearing loose-fitting cotton underwear and changing it frequently; and reducing the intake of dairy products, carbonated beverages, caffeine, and spicy foods.
If symptoms persist, a medical evaluation is necessary to rule out other skin or anal conditions and to prescribe appropriate topical treatments.

Anal itching, medically known as pruritus ani, can be a distressing condition that significantly impacts quality of life. Your symptoms, including alternating constipation and diarrhea, a sensation of incomplete evacuation after bowel movements, and skin irritation around the anal area, suggest a few potential underlying causes that we can explore.
Firstly, the alternating bowel habits you describe could indicate a condition known as irritable bowel syndrome (IBS), which is characterized by chronic abdominal discomfort and changes in bowel habits. IBS can lead to episodes of constipation followed by diarrhea, which may contribute to anal irritation. The frequent wiping due to diarrhea or the straining associated with constipation can cause skin breakdown, leading to the symptoms of itching and pain you are experiencing.
Another common cause of anal itching is skin irritation or dermatitis, which can occur due to excessive moisture, poor hygiene, or even the use of irritating soaps or wipes. When the skin around the anus becomes irritated, it can lead to itching and discomfort. Additionally, if you are experiencing any form of anal fissures—small tears in the skin caused by passing hard stools—this can also lead to pain and itching, especially when wiping.
Infections, such as yeast infections or pinworm infestations, can also cause anal itching. Yeast infections are more common in individuals who have had recent antibiotic use, as antibiotics can disrupt the normal balance of bacteria and yeast in the body. Pinworms, particularly in children, can cause significant itching around the anus, especially at night.
Dietary factors can also play a role in anal itching. Certain foods, such as spicy foods, caffeine, and alcohol, can irritate the gastrointestinal tract and lead to changes in bowel habits, which in turn can exacerbate anal itching. Additionally, inadequate fiber intake can lead to constipation, further contributing to the cycle of irritation.
To address your symptoms, consider the following steps:
1. Dietary Adjustments: Increase your fiber intake through fruits, vegetables, and whole grains to promote regular bowel movements and prevent constipation. Stay hydrated by drinking plenty of water.
2. Hygiene Practices: After bowel movements, gently clean the area with mild soap and water, and pat dry instead of wiping vigorously. Consider using unscented wipes if necessary, but avoid those with alcohol or fragrances.
3. Topical Treatments: Over-the-counter creams or ointments containing hydrocortisone can help reduce inflammation and itching. However, these should be used sparingly and not for prolonged periods without consulting a healthcare provider.
4. Avoid Irritants: Identify and avoid any potential irritants, such as certain soaps, laundry detergents, or toilet paper that may be causing irritation.
5. Consult a Healthcare Provider: If your symptoms persist or worsen, it is essential to consult a healthcare provider for a thorough evaluation. They may perform a physical examination and possibly recommend tests to rule out infections, hemorrhoids, or other underlying conditions.
In summary, anal itching can arise from various causes, including bowel habit changes, skin irritation, infections, and dietary factors. By implementing good hygiene practices, dietary changes, and seeking medical advice when necessary, you can find relief from your discomfort. Remember, it is crucial to address these symptoms early to prevent further complications and improve your overall well-being.
