Ear problems and ear cleaning issues?
Hello doctor, I have recently experienced decreased hearing and occasional ear pain.
I usually use cotton swabs to clean my ears after washing my hair.
I would like to ask if there might be earwax in my ears like in the videos, or if I might have developed otitis media from water entering my ears during swimming lessons.
Can a general ENT clinic help me clean my earwax? Also, do you have any recommendations for external ear cleaning services?

Dr. Zheng Jueyi reply Otolaryngology
Hello, external auditory canal inflammation is common, and general ENT clinics can treat inflammation or clean earwax.
If there is inflammation, it is not recommended to perform ear cleaning.

Common ear problems such as ear pain, hearing loss, and issues related to earwax buildup can significantly affect one's quality of life. It is essential to understand the underlying causes and appropriate management strategies for these conditions.
Firstly, earwax, or cerumen, is a natural substance produced by glands in the ear canal. It serves to protect the ear by trapping dust, debris, and microorganisms, preventing them from reaching the eardrum. However, excessive earwax can lead to blockage, resulting in symptoms such as hearing loss, earache, and a feeling of fullness in the ear. Using cotton swabs to clean the ears is not recommended, as it can push the wax further into the ear canal, potentially leading to impaction and even damage to the ear canal or eardrum.
In your case, you mentioned experiencing occasional ear pain and decreased hearing, which could indeed be related to earwax buildup or possibly an ear infection, especially if you have a history of swimming and water exposure. Water can sometimes lead to conditions like swimmer's ear (otitis externa) or middle ear infections (otitis media), particularly if water remains trapped in the ear canal.
If you suspect that earwax is the issue, it is advisable to consult an ear, nose, and throat (ENT) specialist or visit an otolaryngology clinic. These professionals can perform a thorough examination of your ears and safely remove any excessive wax buildup. They may use specialized instruments or suction devices designed for this purpose, which are much safer than cotton swabs.
Regarding your question about external ear cleaning services, while some places offer ear cleaning, it is crucial to ensure that these services are provided by qualified professionals. Improper cleaning techniques can lead to complications such as perforated eardrums or infections. Therefore, it is best to seek treatment from a licensed healthcare provider who specializes in ear care.
In addition to earwax issues, other common ear problems include:
1. Ear Infections: These can occur in the outer ear (otitis externa) or the middle ear (otitis media). Symptoms may include pain, drainage, and hearing loss. Treatment often involves antibiotics if a bacterial infection is present.
2. Eustachian Tube Dysfunction: This condition occurs when the tube that connects the middle ear to the back of the throat becomes blocked or does not function properly. It can lead to feelings of fullness, discomfort, and fluctuating hearing. Techniques such as the Valsalva maneuver (pinching the nose and gently blowing) can help equalize pressure.
3. Tinnitus: This is characterized by ringing or buzzing in the ears and can be associated with hearing loss, exposure to loud noises, or earwax buildup. Management may involve sound therapy, counseling, or hearing aids.
4. Noise-Induced Hearing Loss: Prolonged exposure to loud sounds can damage the hair cells in the inner ear, leading to permanent hearing loss. It is essential to protect your ears from excessive noise by using earplugs or avoiding loud environments.
In summary, if you are experiencing hearing loss and ear pain, it is crucial to seek professional evaluation from an ENT specialist. They can provide a comprehensive assessment, determine the cause of your symptoms, and recommend appropriate treatment. Avoid using cotton swabs for ear cleaning, and consider professional cleaning if you suspect earwax buildup. Taking proactive steps to care for your ears will help maintain your hearing health and overall well-being.
