Could you please advise if two types of heart medications can be taken together?
Hello, I was recently diagnosed with congenital myocardial bridge.
The doctor prescribed "Isosorbide Mononitrate 40 mg" to be taken twice daily, in the morning and evening.
However, I notice that my heart rate is faster in the mornings, so I regularly take "antiarrhythmic medication" to lower my heart rate.
Can I take Isosorbide Mononitrate and the antiarrhythmic medication together after breakfast? Would this combination potentially cause low blood pressure or an excessively low heart rate? Thank you.

Dr. Wu Xueming reply Cardiology
Both medications have the effect of lowering heart rate.
If taken together, there may be a risk of bradycardia.
Please discuss with your outpatient physician whether to adjust the medication dosage.

More Info
When considering the concurrent use of Isosorbide and heart rate medications, it is essential to understand the pharmacological properties of both drugs and their potential interactions. Isosorbide is a nitrate used primarily for the prevention and treatment of angina pectoris, while heart rate medications, such as beta-blockers (e.g., Propranolol), are commonly prescribed to manage conditions like tachycardia and hypertension.
Safety of Concurrent Use
1. Mechanism of Action: Isosorbide works by dilating blood vessels, which can lead to a decrease in blood pressure. On the other hand, heart rate medications like beta-blockers reduce heart rate and myocardial oxygen demand. While both medications can be beneficial for patients with cardiovascular conditions, their combined effects can lead to hypotension (low blood pressure) or bradycardia (low heart rate).
2. Monitoring: If you are prescribed both Isosorbide and a heart rate medication, it is crucial to monitor your blood pressure and heart rate regularly. If you notice symptoms such as dizziness, lightheadedness, or excessive fatigue, these could be signs of hypotension or bradycardia, and you should consult your healthcare provider immediately.
3. Consultation with Healthcare Provider: Before making any changes to your medication regimen, it is vital to discuss your concerns with your healthcare provider. They can assess your specific situation, including your overall health, the severity of your heart condition, and any other medications you may be taking. They may recommend adjusting dosages or changing medications based on your response.
Potential Risks
1. Low Blood Pressure: The combination of Isosorbide and heart rate medications can lead to an additive effect on blood pressure reduction. This is particularly concerning if you are already on other antihypertensive medications.
2. Bradycardia: If both medications lower your heart rate significantly, it could lead to bradycardia, which may cause symptoms such as fatigue, weakness, or fainting.
3. Individual Variability: Each patient responds differently to medications. Factors such as age, weight, kidney function, and the presence of other medical conditions can influence how these drugs interact in your body.
Recommendations
- Timing of Doses: If your healthcare provider approves the concurrent use of Isosorbide and heart rate medication, they may suggest specific timing for taking these medications to minimize potential interactions. For example, taking them at different times of the day may help reduce the risk of low blood pressure or heart rate.
- Lifestyle Modifications: Alongside medication management, consider lifestyle changes that can help manage your heart condition. This includes a heart-healthy diet, regular exercise (as advised by your doctor), and stress management techniques.
- Regular Follow-ups: Schedule regular follow-ups with your healthcare provider to monitor your heart condition and medication effects. This will allow for timely adjustments to your treatment plan based on your progress and any side effects you may experience.
In summary, while it may be safe to take Isosorbide and heart rate medications together, it is crucial to do so under the guidance of your healthcare provider. Regular monitoring and open communication with your doctor will help ensure that your treatment is both safe and effective.
