Post-operative issues with bowel movements after hemorrhoid surgery?
I underwent a hemorrhoid surgery using a tissue coagulation knife on May 9th, and now it's the 8th day.
I have been having bowel movements more than 6 or 7 times a day, and each time is very painful.
I constantly feel the urge to go, but often only a small amount comes out.
Is this normal? What can I do to reduce the frequency of my bowel movements? How long will it take to return to my previous pattern of having 1 or 2 bowel movements a day? Thank you, Dr.
Zhang.

Post-operative difficulty in bowel movements after hemorrhoid surgery is mostly caused by pain preventing relaxation.
Adjusting pain medication, taking warm sitz baths, maintaining a light diet, and increasing fluid intake can help.
Improvement is typically seen about two weeks after the wound heals.
If discomfort persists during this period, it is important to return to the surgeon for evaluation.

After undergoing hemorrhoid surgery, it is not uncommon to experience changes in bowel habits and discomfort during bowel movements. Your situation, where you are having bowel movements 6 to 7 times a day with significant pain, is concerning but can be understood within the context of the healing process and the surgical intervention you received.
Firstly, it’s important to recognize that after hemorrhoid surgery, the surgical site is healing, and this can lead to increased sensitivity and discomfort during bowel movements. The pain you are experiencing could be due to several factors, including inflammation at the surgical site, the presence of stitches, or even the formation of scar tissue. Additionally, the urge to have frequent bowel movements may be a result of your body adjusting to the changes post-surgery, as well as potential dietary factors.
Here are some strategies to help manage your symptoms and improve your bowel habits:
1. Dietary Adjustments: Focus on a high-fiber diet to help soften your stools and make bowel movements easier. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying well-hydrated is also crucial, as adequate fluid intake can help prevent constipation and reduce straining during bowel movements.
2. Stool Softeners: Over-the-counter stool softeners can be beneficial in easing the passage of stools. These medications work by increasing the water content in your stool, making it softer and easier to pass. Consult your doctor about which stool softener might be appropriate for you.
3. Warm Baths: Taking warm baths can help soothe the area and reduce discomfort. This practice, often referred to as sitz baths, can promote relaxation and may help with pain management.
4. Pain Management: If you are experiencing significant pain, discuss with your healthcare provider about appropriate pain relief options. This may include over-the-counter pain medications or prescribed medications that can help manage your discomfort.
5. Gradual Return to Normal Bowel Habits: It’s important to allow your body time to heal. While it may take some time to return to your normal bowel habits of 1-2 times a day, most patients find that their bowel patterns stabilize within a few weeks to a couple of months after surgery. However, if your symptoms persist or worsen, it’s essential to follow up with your healthcare provider.
6. Avoid Straining: Straining during bowel movements can exacerbate pain and lead to complications. If you feel the urge to go but are unable to pass a stool, try to relax and wait for a more opportune moment.
7. Follow-Up Care: Regular follow-up appointments with your surgeon or healthcare provider are crucial. They can assess your healing progress and address any complications that may arise, such as infection or excessive scarring.
In summary, while it is not unusual to experience pain and changes in bowel habits after hemorrhoid surgery, it is essential to manage your symptoms proactively. Implementing dietary changes, using stool softeners, and maintaining open communication with your healthcare provider can significantly improve your recovery experience. If your symptoms do not improve or if you have concerns about your healing process, do not hesitate to reach out to your doctor for further evaluation and guidance.
