Can the installation of a maxillary expander improve symptoms of nasal and ocular allergies?
Hello, Doctor Zeng.
I am currently 21 years old and was diagnosed with ankylosing spondylitis at the age of 17.
Additionally, I have experienced allergic rhinitis with symptoms such as a runny nose in response to changes in temperature and humidity since childhood, as well as red and itchy eyes due to allergies.
In late October 2023, I had an upper jaw expander placed due to orthodontic treatment, and since then, my upper dental arch has expanded significantly.
From November 2023 to April 2024, which is winter, there have been considerable changes in temperature and humidity, yet I have not experienced any allergic reactions.
I would like to inquire whether the use of the upper jaw expander could have improved my nasal and ocular allergy symptoms.
Thank you for your assistance!

Dr. Zeng Guosen reply Internal Medicine
There is no evidence to suggest that the use of a maxillary expander can improve symptoms of nasal and ocular allergies.
It is recommended to consult with a dentist or an otolaryngologist for further advice.
I apologize for any shortcomings in my knowledge.
Thank you for your question.

More Info
Palatal expanders are orthodontic devices used primarily to widen the upper jaw (maxilla) to create more space for teeth alignment. However, there is emerging interest in their potential effects on nasal and eye allergy symptoms, particularly in individuals with a history of allergies or respiratory issues.
In your case, it seems that since the installation of the palatal expander, you have experienced a reduction in your allergy symptoms, such as nasal congestion and itchy, red eyes. This observation raises an interesting question about the relationship between dental structures, the nasal cavity, and allergy symptoms.
Mechanism of Action
The palatal expander works by physically separating the two halves of the maxilla, which can lead to an increase in the width of the nasal cavity. This widening may improve airflow through the nasal passages, potentially reducing nasal congestion and the likelihood of allergic reactions. When the nasal passages are more open, it can lead to better drainage of mucus and less irritation from allergens, which could explain the alleviation of your symptoms.
Moreover, the anatomy of the nasal cavity is closely linked to the oral cavity. The expansion of the palate can also influence the position of the tongue and the overall oral posture, which may further affect breathing patterns. Improved nasal breathing can reduce mouth breathing, which is often associated with allergies and can exacerbate symptoms.
Clinical Evidence
While there is anecdotal evidence and some preliminary studies suggesting that palatal expansion may help alleviate symptoms of nasal allergies, more extensive clinical research is needed to establish a definitive link. Some studies have indicated that children with allergic rhinitis may benefit from orthodontic treatments that include palatal expansion, but the results can vary widely among individuals.
Considerations
1. Duration of Improvement: It is essential to monitor whether the improvement in your allergy symptoms is sustained over time. Seasonal changes, exposure to allergens, and other environmental factors can also play a significant role in allergy symptoms.
2. Underlying Conditions: Given your history of ankylosing spondylitis and sensitivity to temperature and humidity changes, it is crucial to consider that your overall health and immune response may also influence your allergy symptoms.
3. Consultation with Specialists: If your allergy symptoms return or worsen, it may be beneficial to consult with an allergist or an ENT specialist. They can provide a comprehensive evaluation and may suggest additional treatments, such as antihistamines or nasal corticosteroids, if necessary.
4. Orthodontic Follow-Up: Continue to follow up with your orthodontist regarding the palatal expander. They can monitor the expansion process and its effects on your oral and nasal health.
Conclusion
In summary, while there is a plausible mechanism by which palatal expanders could alleviate nasal and eye allergy symptoms, individual responses can vary. Your experience of reduced allergy symptoms since the installation of the expander is promising, but it is essential to remain vigilant and consult with healthcare professionals if any concerns arise. Continued monitoring and a multidisciplinary approach involving orthodontics, allergy management, and possibly ENT evaluation will provide the best outcomes for your health.
