Menstrual irregularities after discontinuing antidepressants?

Hello, doctor.
I have a question: I started taking antidepressants (Fluoxetine) in early February of this year.
My period arrived on February 14, but it was brown in color and very light, lasting only until the 16th.
I did not have my period in March or April.
I stopped taking the medication at the end of April or early May, and as of May 22, I still have not had my period.
It has been a while since I stopped the medication.
Is this normal, or could it be a side effect of the medication?

Chun, 20~29 year old female. Ask Date: 2024/05/22

Hello: Psychiatric medications can affect menstruation, and it is recommended to visit an obstetrician-gynecologist for a detailed examination.

When it comes to the missed period after stopping antidepressants, it is essential to understand that hormonal changes can be influenced by various factors, including medication, stress, and overall health. In your case, you mentioned starting an antidepressant (specifically, fluoxetine, known as "舒必朗") in February, and since then, you have experienced irregular menstrual cycles, including a very light period in February and no periods in March and April. After discontinuing the medication in late April or early May, you still have not had a period by May 22.

Firstly, it is not uncommon for antidepressants to affect menstrual cycles. Selective serotonin reuptake inhibitors (SSRIs), like fluoxetine, can influence hormonal balance, which may lead to changes in menstrual patterns. Some women may experience lighter periods, missed periods, or even changes in the timing of their cycles while on these medications. This is often due to the way these drugs interact with neurotransmitters that can also affect the hypothalamus and pituitary gland, which play crucial roles in regulating the menstrual cycle.

After stopping the medication, it can take some time for your body to readjust and for your menstrual cycle to return to its normal rhythm. The duration of this adjustment period can vary significantly from person to person. Some may find their cycles normalize within a few weeks, while others may take several months to regain regularity. Factors such as stress, changes in weight, lifestyle, and overall health can also play a significant role in how quickly your menstrual cycle returns to normal.

In your case, since you have not had a period for several months, it is advisable to consult with your healthcare provider. They can help determine whether your missed periods are a result of stopping the medication or if there may be other underlying issues that need to be addressed. It is also important to rule out pregnancy if you are sexually active, as this could be a contributing factor to your missed periods.

In summary, while it is not unusual for antidepressants to affect menstrual cycles, the absence of your period for an extended period after stopping the medication warrants further evaluation. Your healthcare provider can offer guidance and may recommend monitoring your menstrual cycle for a few more weeks or conducting tests to rule out other potential causes. Additionally, maintaining a healthy lifestyle, managing stress, and ensuring proper nutrition can support hormonal balance and overall well-being during this transition.
