Does diarrhea smell worse than usual bowel movements?

Hello doctor, I usually experience constipation, so my stool tends to have a stronger odor than average.
However, I have been monitoring the color and shape of my stool, and it consistently has a similar smell.
I don't often have diarrhea, maybe just once or twice in six months due to eating something bad.
Recently, I've noticed that when I do have more severe diarrhea (liquid stools), the odor is extremely foul.
My questions are: 1.
Is it normal for diarrhea to have a very strong odor? 2.
I had a colonoscopy three years ago, and the results were normal with no polyps.
How often should I have another colonoscopy?

Dr. Chen Shidian reply Gastroenterology and Hepatology

The odor is inevitable and unrelated to the polyps.
Be sure to stay hydrated.

Diarrhea can indeed have a stronger odor compared to normal bowel movements, and this is often due to several factors related to the underlying causes of diarrhea. When the digestive system is functioning normally, the body is able to break down food efficiently, and the waste produced is typically less odorous. However, when diarrhea occurs, it can be a sign that the body is not absorbing nutrients properly, which can lead to the presence of unabsorbed food particles and bacteria in the stool. This can result in a more pungent smell.

1. Is it normal for diarrhea to have a foul odor?
Yes, it is normal for diarrhea to have a stronger and more unpleasant odor than regular stool. This can be attributed to several factors:
- Increased Bacterial Activity: Diarrhea often results from infections (viral, bacterial, or parasitic) that can alter the gut flora, leading to an increase in gas production and foul-smelling waste.

- Undigested Food: When food passes through the intestines too quickly, it may not be fully digested, leading to the presence of undigested food particles in the stool, which can contribute to a stronger odor.

- Fat Malabsorption: Conditions that affect fat absorption, such as celiac disease or pancreatitis, can lead to steatorrhea (fatty diarrhea), which is often characterized by a particularly foul smell.

- Dietary Factors: Certain foods, such as those high in sulfur (like garlic and cruciferous vegetables), can also contribute to a stronger odor in stool.

If you notice a sudden change in the odor of your stool, especially if accompanied by other symptoms like abdominal pain, fever, or blood in the stool, it is advisable to consult a healthcare professional.

2. How often should a colonoscopy be performed if the last one was normal?
The frequency of colonoscopy screenings can vary based on individual risk factors, family history, and the findings of previous examinations. Generally, if your last colonoscopy was normal and you have no significant risk factors for colorectal cancer (such as a family history of the disease or certain genetic conditions), the American College of Gastroenterology recommends that you undergo a screening colonoscopy every 10 years starting at age 45 for average-risk individuals. However, if you have a history of polyps or other risk factors, your doctor may recommend more frequent screenings.

It is essential to maintain open communication with your healthcare provider regarding your gastrointestinal health. If you have concerns about your bowel movements, whether it be odor, consistency, or frequency, or if you experience any new or worsening symptoms, it is crucial to seek medical advice. Regular check-ups and screenings are vital for early detection and prevention of potential gastrointestinal issues.
