Postoperative bleeding after septoplasty?
Hello, Doctor.
I previously went to the ENT department for an examination and found that I have a deviated septum and hypertrophy of the inferior turbinate.
I underwent surgery for thick rhinitis and deviated septum on June 4th, using self-dissolving packing.
Today is the fifth day post-surgery, and I feel a sensation of fluid and foreign body in my throat, so I tried to clear my throat.
Unfortunately, I accidentally inhaled too forcefully through my nose, which caused it to start bleeding.
At that moment, I used sterile cotton balls and applied ice for 10 minutes, but there is still a small amount of bleeding.
I would like to ask the following questions: 1.
How should I manage this moving forward? 2.
Will this affect the surgical site? 3.
Can I continue to use a nasal irrigator? Thank you, Doctor.

Dr. Zheng Jueyi reply Otolaryngology
If there is only a small amount of bleeding, you can insert a sterile cotton ball and apply pressure to the front of the nose to help stop the bleeding, along with continuous ice packs.
However, if the bleeding persists, it is advisable to return to the hospital for outpatient or emergency treatment.
It is temporarily not recommended to use a nasal irrigator.
Generally, you should wait until the wound is stable, about 1-2 weeks after surgery, before rinsing the nose to avoid further bleeding.

Post-surgery complications, particularly after a septoplasty, can be concerning for patients. It's important to understand the healing process and how to manage any complications that arise. In your case, experiencing bleeding after suctioning your nose is not uncommon, especially within the first week post-surgery. Here’s a detailed response to your questions based on medical knowledge and practices.
1. How to Manage the Bleeding:
If you experience a nosebleed after surgery, the first step is to remain calm. You can follow these steps:
- Sit upright and lean slightly forward to prevent blood from flowing down the back of your throat.
- Pinch your nostrils together for about 10-15 minutes. This pressure can help stop the bleeding.
- Apply a cold compress or ice pack to the bridge of your nose to constrict blood vessels and reduce swelling.
- Avoid blowing your nose or engaging in any activities that could increase pressure in your nasal passages, such as heavy lifting or vigorous exercise.
- If the bleeding persists for more than 20 minutes or is heavy, you should seek medical attention immediately.
2. Impact on the Surgical Site:
Minor bleeding is often a normal part of the healing process after nasal surgery. However, if you notice a significant increase in bleeding or if it becomes recurrent, it could indicate that the surgical site has been disturbed or that there is an issue with the healing process. It's essential to monitor the amount and frequency of the bleeding. If you have concerns, it’s best to consult your surgeon or an ENT specialist to assess the situation and ensure that the surgical site is healing properly.
3. Using a Nasal Irrigation Device:
After septoplasty, nasal irrigation can be beneficial for keeping the nasal passages clear of mucus and promoting healing. However, it is crucial to wait until your doctor gives you the green light to resume this practice. Typically, surgeons recommend waiting at least a week or until the initial healing phase is complete before using a nasal rinse. If you are experiencing bleeding, it may be best to avoid nasal irrigation until you have consulted with your healthcare provider. They can provide specific guidance based on your healing progress and any complications you may be experiencing.
In addition to these points, it’s important to maintain follow-up appointments with your ENT specialist. They can monitor your recovery and address any concerns you may have. If you experience symptoms such as increased pain, swelling, fever, or persistent bleeding, these could be signs of infection or other complications that require prompt medical attention.
Lastly, remember that healing after nasal surgery can take time, and it’s normal to experience some discomfort and changes in sensation during the recovery period. Patience and adherence to your surgeon's post-operative care instructions are key to a successful recovery. If you have any further questions or concerns, don’t hesitate to reach out to your healthcare provider for personalized advice.
