Three months after hemorrhoid surgery?

Hello doctor, I had hemorrhoid surgery (using ultrasound knife) to remove mixed hemorrhoids three months ago.
However, in the past few days, while cleaning my anus, I felt what seems to be protruding tissue that resembles hemorrhoids.
I've also experienced some swelling after standing for too long.
I would like to know if it's possible for hemorrhoids to recur shortly after surgery? (I have poor blood circulation.)

Lìzhēn, 40~49 year old female. Ask Date: 2024/06/13

Dr. Zhang Shuhao reply Surgery

Regardless of the type of hemorrhoid surgery performed, recurrence is unlikely within three months.
It is advisable to take warm sitz baths to help reduce swelling.
If symptoms persist, it is recommended to return to the surgeon for a follow-up appointment to examine the surgical site and adjust medications, ensuring the underlying cause of the symptoms is identified.

Hemorrhoids, also known as piles, can indeed recur after surgical intervention, even within a few months post-procedure. The recurrence of hemorrhoids after surgery can be influenced by several factors, including the type of surgery performed, the underlying causes of the hemorrhoids, and individual patient factors such as lifestyle, diet, and pre-existing medical conditions.

In your case, having undergone a hemorrhoidectomy using ultrasound technology to remove mixed hemorrhoids, it is not uncommon to experience some residual symptoms or even the development of new hemorrhoids in the months following the surgery. The fact that you are feeling a bulging sensation and experiencing swelling after prolonged standing could indicate a few possibilities:
1. Residual Hemorrhoidal Tissue: Sometimes, not all hemorrhoidal tissue is completely removed during surgery, which can lead to the sensation of a recurrence.
2. New Hemorrhoids: It is possible to develop new hemorrhoids after surgery, especially if the underlying causes (such as straining during bowel movements, prolonged sitting or standing, or poor dietary habits) are still present.

3. Post-Surgical Changes: After surgery, the area may undergo changes that can lead to discomfort or swelling, which might mimic the symptoms of hemorrhoids.

4. Circulatory Issues: You mentioned having poor blood circulation, which can exacerbate issues related to hemorrhoids. Poor circulation can lead to increased pressure in the rectal area, contributing to the formation of new hemorrhoids.

To address your concerns, here are some recommendations:
- Follow-Up Consultation: It is crucial to consult your surgeon or a gastroenterologist for a thorough examination. They can assess whether what you are feeling is indeed a recurrence of hemorrhoids or if it is related to post-surgical changes.

- Dietary Adjustments: Ensure that your diet is high in fiber to prevent constipation and straining during bowel movements. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Staying well-hydrated is also essential.

- Lifestyle Modifications: Avoid prolonged sitting or standing, and try to incorporate regular physical activity into your routine to improve circulation and reduce pressure in the rectal area.

- Pain Management: If you are experiencing discomfort, over-the-counter pain relievers may help, but consult your doctor before taking any medication.

- Monitoring Symptoms: Keep track of any changes in your symptoms, such as increased pain, bleeding, or changes in bowel habits, and report these to your healthcare provider.

In summary, while it is possible for hemorrhoids to recur after surgery, the exact cause of your symptoms should be evaluated by a healthcare professional. They can provide personalized advice and treatment options based on your specific situation. Regular follow-up appointments and proactive management of your diet and lifestyle can significantly reduce the risk of recurrence and help maintain your overall rectal health.
