Frenulum Concerns After Circumcision: What to Expect - Urology

Issues related to the frenulum of the foreskin?

Hello Doctor, I had a circumcision three weeks ago, and I still have two stitches that haven't fallen out.
I would like to ask if it's normal for me to see my frenulum and the entire glans while in a non-erect state, since I previously had phimosis and wasn't aware of this.
When I have an erection, the frenulum gets taut; does this mean it could easily tear during sexual activity or masturbation if it is exposed? Additionally, I've heard that stitches usually fall out on their own within two to three weeks.
Is it normal for me to still have some stitches at this point?

A Jun, 20~29 year old female. Ask Date: 2024/06/24

Dr. Du Shixi reply Urology

Hello: What you mentioned is normal; the ligaments do not require special treatment.
Please return for a follow-up regarding the stitches.
Wishing you good health.

After undergoing circumcision, it's common to have concerns regarding the healing process and the appearance of the frenulum and glans (the head of the penis). Let's address your questions and provide some insights into what you can expect during your recovery.

Firstly, seeing your frenulum and glans while in a non-erect state is entirely normal after circumcision. The frenulum, which is the band of tissue on the underside of the penis that connects the glans to the shaft, may be more visible post-surgery, especially if you had a condition like phimosis (tight foreskin) before the procedure. The surgical removal of the foreskin exposes the glans and the frenulum, which is a typical outcome of the circumcision process.

Regarding your concern about the frenulum being taut during an erection, it’s important to note that while the frenulum does stretch, it is designed to handle this tension. The risk of the frenulum tearing during sexual activity or masturbation is generally low, especially if the circumcision was performed correctly and the area is healing well. However, if you experience any pain, discomfort, or unusual sensations during erections or sexual activity, it would be wise to consult your healthcare provider for further evaluation.

As for the sutures, it is common for absorbable stitches to take anywhere from one to three weeks to dissolve. The timeline can vary based on individual healing rates, the type of sutures used, and how well you are following post-operative care instructions. If you still have sutures after three weeks, it’s not necessarily a cause for concern, but it is advisable to follow up with your surgeon or urologist. They can assess whether the sutures are dissolving as expected or if they need to be removed manually.

In terms of post-operative care, ensure you are keeping the area clean and dry, as this promotes healing and reduces the risk of infection. Avoiding strenuous activities and sexual intercourse until you receive clearance from your doctor is also crucial for a smooth recovery.

Lastly, if you have any concerns about the appearance of your frenulum or glans, or if you notice any signs of infection (such as increased redness, swelling, or discharge), do not hesitate to reach out to your healthcare provider. They can provide reassurance, address your concerns, and ensure that your recovery is progressing as it should.

In summary, it is normal to see your frenulum and glans after circumcision, and the healing process can vary from person to person. If you have lingering concerns about your sutures or any discomfort, a follow-up appointment with your healthcare provider is recommended. They can provide personalized advice and ensure that everything is healing properly.
