More than a month after surgery for anal abscess?

Hello! I had surgery for an anal abscess over a month ago, and I've been taking sitz baths and applying medication daily.
However, currently, the affected area feels like there is a bag with a pressing sensation and a watery sound, along with a small amount of pus draining out.
I find that using wet wipes does not help in cleaning the area.
Is this considered normal?

Mr. He, 30~39 year old female. Ask Date: 2024/06/28

Dr. Zhang Shuhao reply Surgery

Generally, the condition after an anal abscess drainage surgery stabilizes within four to six weeks.
However, due to factors such as the size and depth of the abscess, the degree of soft tissue inflammation, and individual differences in immune response, healing may take longer.
Mild drainage of pus in the early postoperative period may be normal, but if there is a persistent sensation of fluid pressure for over a month, it is important to check for any residual abscess or infection that may not have been adequately drained.
Here are some recommendations: 1.
Continue sitz baths and medication: Maintain daily sitz baths and use the medications prescribed by your doctor, while also monitoring any changes in the wound.
2.
Maintain cleanliness: Keep the affected area clean to prevent further infection.
3.
Seek medical attention promptly: It is advisable to return for a follow-up appointment as soon as possible, inform your doctor of your condition, and undergo a thorough examination to determine if further treatment or medication adjustments are necessary.

After undergoing surgery for an anal abscess, it is not uncommon to experience a range of symptoms during the recovery process. Your description of feeling a "bag-like" sensation with a pressing sound and the presence of a small amount of pus is concerning and warrants further evaluation.
Firstly, it is important to understand that after surgery for an anal abscess, the area may still be healing, and some fluid drainage can be expected. However, the presence of pus is a sign that there may be an ongoing infection or that the abscess has not completely resolved. In a typical recovery, you should see a gradual decrease in pain, swelling, and drainage as the wound heals. If you are experiencing persistent drainage, especially if it is accompanied by pain, redness, or fever, it could indicate that the abscess has not fully drained or that there is a new infection.

The sensation of a "bag" or pressure could be due to several factors. It might be related to the healing process where the tissue is still inflamed or there could be a collection of fluid (seroma) or pus that has not been adequately drained. In some cases, a fistula may develop, which is an abnormal connection between the anal canal and the skin, leading to ongoing drainage and discomfort.
To manage your symptoms, continue with sitz baths as they can help soothe the area and promote healing. Keeping the area clean and dry is crucial. However, it is essential to consult your surgeon or healthcare provider about your symptoms. They may recommend imaging studies, such as an ultrasound or MRI, to assess the area for any complications like abscess recurrence or fistula formation.
In terms of treatment, if there is an infection, your doctor may prescribe antibiotics. If there is a significant collection of pus, a minor surgical procedure may be necessary to drain it.
In summary, while some drainage can be normal after anal abscess surgery, the presence of pus and the sensations you are experiencing should be evaluated by a healthcare professional. Prompt attention to these symptoms can help prevent complications and ensure a smoother recovery. Always trust your instincts; if something feels off, it’s best to seek medical advice.
