Hello, Doctor.
I would like to ask about medication.
About one or two months ago, I visited a psychiatrist due to issues with attention and concentration.
After listening to my description of the situation, the doctor prescribed me Atomoxetine.
Initially, I took one capsule a day for a week and felt some improvement.
During my second follow-up appointment, the doctor suggested I try one and a half capsules.
After taking it for about two weeks to almost a month, I experienced significant side effects, including dry heaving and nausea, which severely affected my work performance.
Due to my discomfort, I stopped taking the medication for a while.
Although the symptoms are not as intense as before, I still feel nauseous and have dry heaving.
How should I manage this feeling of nausea? Additionally, are there any other medications besides Atomoxetine that are less intense but still effective?
Regarding my attention issues, I noticed them after graduating from university and entering the workforce.
I often forget things at work, and when asked what I did today, I struggle to recall it easily, sometimes even remembering incorrectly.
I tend to mix up tasks and find it challenging to logically connect unrelated thoughts.
It is difficult for me to understand instructions or organize my work effectively.
When I encounter problems, I easily become anxious and feel stuck, not knowing what to do next.
Is this what is referred to as attention deficit? Also, what should I do about the side effects caused by Atomoxetine? Can you recommend any over-the-counter medications other than Atomoxetine?

Hello.
Amphetamine-based medications such as Adderall or lisdexamfetamine can be used for ADHD.
The side effects are generally similar.
Nausea and discomfort can be alleviated by taking them with food.
Additionally, experiencing anxiety in challenging situations is not strongly related to ADHD.
If side effects are intolerable, it is advisable to return to the clinic to discuss with the physician the possibility of changing medications or adjusting the dosage.

More Info
Managing attention issues, especially when they are compounded by medication side effects, can be quite challenging. It sounds like you have been experiencing significant difficulties with concentration and memory, which have been exacerbated by the side effects of Abilify (aripiprazole). Let’s break down your concerns and explore potential solutions.
Understanding Your Symptoms
From your description, it appears that you are experiencing symptoms consistent with attention deficit issues, which can manifest as forgetfulness, difficulty in maintaining focus, and challenges in logical reasoning. These symptoms can be particularly pronounced in high-pressure environments, such as the workplace, where the demands for attention and organization are heightened.
The side effects you are experiencing from Abilify, such as nausea and dry heaving, are not uncommon. These side effects can significantly impact your quality of life and your ability to perform at work. It’s important to communicate these side effects to your prescribing physician, as they may need to adjust your dosage or consider alternative medications.
Managing Medication Side Effects
1. Consult Your Doctor: The first step is to discuss your side effects with your psychiatrist. They may suggest adjusting the dosage or switching to a different medication that may have a more tolerable side effect profile.
2. Timing and Food: Sometimes, taking medication with food can help mitigate nausea. You might want to try taking your medication with a light meal or snack to see if that alleviates some of the discomfort.
3. Hydration and Ginger: Staying hydrated and consuming ginger (in the form of tea or candies) can also help reduce nausea.
4. Gradual Dose Adjustment: If your doctor agrees, they might suggest a more gradual increase in dosage to help your body adjust to the medication.
Alternatives to Abilify
If Abilify is not suitable for you due to side effects, there are several alternatives that you can discuss with your doctor:
1. Stimulants: Medications like methylphenidate (Ritalin) or amphetamines (Adderall) are commonly used to treat attention deficit hyperactivity disorder (ADHD) and can improve focus and concentration. However, they may also have side effects, so a thorough discussion with your doctor is essential.
2. Non-Stimulant Options: Atomoxetine (Strattera) is a non-stimulant medication that is often used for ADHD and may have a different side effect profile compared to stimulants and antipsychotics.
3. Other Atypical Antipsychotics: Medications like quetiapine (Seroquel) or lurasidone (Latuda) may be considered, but again, this should be done under the guidance of your healthcare provider.
Coping Strategies for Attention Issues
In addition to medication, there are several strategies you can implement to help manage your attention issues:
1. Structured Environment: Create a structured work environment with clear lists and priorities. Use tools like calendars, reminders, and task management apps to keep track of your responsibilities.
2. Mindfulness and Relaxation Techniques: Practices such as mindfulness meditation, deep breathing exercises, or yoga can help reduce anxiety and improve focus.
3. Break Tasks into Smaller Steps: Breaking down tasks into smaller, manageable steps can make them feel less overwhelming and help you maintain focus.
4. Regular Breaks: Taking regular breaks can help refresh your mind and improve overall productivity. The Pomodoro Technique, which involves working for 25 minutes followed by a 5-minute break, can be particularly effective.
5. Physical Activity: Regular exercise has been shown to improve concentration and cognitive function. Even short walks can help clear your mind and improve focus.
Conclusion
It’s crucial to address both the medication side effects and the underlying attention issues you are experiencing. Open communication with your healthcare provider is key to finding the right balance of medication and coping strategies. Remember, managing attention issues is often a multifaceted approach that includes medication, behavioral strategies, and lifestyle changes. Don’t hesitate to seek support from mental health professionals who can provide additional resources and guidance tailored to your specific needs.
