There is a bony protrusion beneath the right costal cartilage?

Hello, Doctor.
About two months ago, I started doing crunches as part of my fitness routine and occasionally noticed a sensation in my right upper abdomen, as if some tissue was being moved (it’s not painful).
At that time, I took a deep breath and felt a hard protrusion near the costal cartilage close to the sternum on the right side (it feels like a bony lump that only causes slight discomfort when pressed firmly).
So far, I haven't experienced any discomfort in my body, and this sensation only occurs occasionally when I engage my upper abdominal muscles, and the protrusion is still present.
Could you please advise if this might indicate any medical condition or if treatment is necessary? Or could it possibly be a congenital issue? Thank you for taking the time to respond!

Dr. Shi Guozheng reply Orthopedics

It could possibly be a sternal protrusion, so it's best to go to the hospital for an X-ray examination.

Understanding a hard bump below the right rib can be concerning, especially when it is associated with physical activity like exercising. Based on your description, it seems that you have noticed a hard, possibly bony protrusion in the upper right abdomen, particularly when performing abdominal exercises. This bump is not causing you pain, but you do feel some discomfort when pressing on it.

There are several potential explanations for the hard bump you are experiencing. One possibility is that it could be a normal anatomical variation or a benign condition such as a costochondral junction prominence. The costochondral junction is where the ribs meet the cartilage, and sometimes, due to muscle development or changes in body composition from exercise, this area can become more pronounced.
Another possibility could be related to muscle hypertrophy or changes in the abdominal wall due to your workout regimen. As you engage in exercises like crunches, the muscles in your abdomen, including the rectus abdominis, can become more developed, which may lead to a feeling of tightness or the sensation of a hard bump.
However, it is also essential to consider other conditions that could cause a hard mass in that area. For instance, a hernia could present as a bulge or lump, especially if it becomes more noticeable during physical activity or when straining. Inguinal hernias, for example, can sometimes manifest in the upper abdomen, although they are more commonly found in the groin area.
Additionally, other conditions such as lipomas (benign fatty tumors), fibromas, or even more serious concerns like tumors should not be ruled out entirely. While these are less common, they can present as hard masses in the abdominal area.

Given that you have not experienced any significant discomfort or other symptoms, it may not be an urgent concern. However, it is crucial to monitor the bump for any changes in size, shape, or associated symptoms such as pain, swelling, or changes in bowel habits. If you notice any of these changes, it would be wise to seek medical evaluation.

In terms of treatment, if the bump is determined to be benign and not causing any issues, it may not require any intervention. However, if it is found to be a hernia or another condition that requires treatment, your healthcare provider will discuss the appropriate options with you.

In conclusion, while the hard bump below your right rib may be benign, it is essential to have it evaluated by a healthcare professional, especially if it persists or changes over time. A physical examination and possibly imaging studies, such as an ultrasound or CT scan, may be necessary to determine the exact nature of the bump. Always prioritize your health and seek medical advice when in doubt.
