Which hospital should I go to for treatment?

Hello, my girlfriend went to the hospital for an examination and was diagnosed with a condition commonly known as "cauliflower" (genital warts).
I have noticed small red spots at the edge of my glans.
Since I had unprotected sexual intercourse with my girlfriend, I believe I may have contracted the same condition.

Currently, the methods introduced online include interferon injections, electrosurgery, cryotherapy, and immune response modifiers (IRM) such as Aldara cream.
Due to the potential for pain, infection, and scarring associated with electrosurgery and cryotherapy, both my girlfriend and I are considering interferon injections or applying the immune response modifier (IRM) Aldara cream.

Could you please advise which hospitals under the Department of Health offer these treatments? Thank you.

Kunrao de ren, 30~39 year old female. Ask Date: 2004/03/12

Dr. Chen Xiuyi reply Urology

Hello: Before treating genital warts, it is essential to confirm the diagnosis.
Since your girlfriend has genital warts, your risk of transmission is higher.
However, it is still necessary to confirm the diagnosis before treatment.
Common treatment options include electrocautery, cryotherapy, or topical medications.
Interferon is administered via injection for treatment.
In terms of effectiveness, topical treatments like Aldara (an immune modulator) or interferon injections typically require a longer treatment duration and are not covered by health insurance.
Aldara costs over 300 NTD for a small package and must be paid out of pocket.
Sincerely, Dr.
Lu Li-Cheng, Department of Obstetrics and Gynecology, Taoyuan Hospital, Ministry of Health and Welfare.

When it comes to managing HPV (Human Papillomavirus) infections and related conditions, it is essential to understand the nature of the virus, its potential health implications, and the available treatment options. In your case, both you and your girlfriend have been diagnosed with HPV-related conditions, which understandably raises concerns about health outcomes and treatment options.

Understanding HPV and Its Implications
HPV is a common sexually transmitted infection, with many types that can affect the genital area, mouth, and throat. While some types of HPV can lead to genital warts (often referred to colloquially as "cauliflower" or "菜花"), others are associated with a higher risk of developing cervical cancer and other cancers. It's important to note that not everyone who is infected with HPV will develop symptoms or health problems. In many cases, the immune system can clear the virus without any intervention.

Treatment Options
1. Monitoring and Follow-Up: Regular follow-up with a healthcare provider is crucial. This typically includes Pap smears and HPV testing to monitor any changes in cervical cells. If your girlfriend has been diagnosed with mild cervical dysplasia, it is essential to follow up as recommended by her healthcare provider.

2. Topical Treatments: For external genital warts, treatments may include topical medications such as Imiquimod (an immune response modifier) or Podofilox. These can help in reducing the warts without the need for invasive procedures.

3. Procedural Treatments: If warts are extensive or do not respond to topical treatments, procedures such as cryotherapy (freezing), electrosurgery (burning), or laser treatment may be considered. While these methods can be effective, they may come with risks of pain, scarring, or infection.

4. Interferon Therapy: Interferon therapy has been used in some cases to treat HPV-related conditions, particularly for persistent warts. This treatment aims to boost the immune response against the virus.

5. Vaccination: The HPV vaccine (Gardasil or Cervarix) can protect against the most common high-risk HPV types and is recommended for individuals up to age 26, and in some cases, up to age 45. If you or your girlfriend have not been vaccinated, it may be worth discussing with a healthcare provider.

Where to Seek Treatment in Taiwan
In Taiwan, various hospitals and clinics offer specialized care for HPV-related conditions. The Ministry of Health and Welfare provides a list of accredited hospitals where you can seek treatment. Here are some steps to find appropriate care:
1. Consult a Gynecologist or Urologist: Since HPV primarily affects the genital area, consulting a gynecologist for your girlfriend and a urologist for yourself would be beneficial. They can provide tailored advice and treatment options based on your specific conditions.

2. Hospital Resources: Major hospitals in Taiwan, such as National Taiwan University Hospital, Taipei Veterans General Hospital, and Chang Gung Memorial Hospital, have departments specializing in infectious diseases, dermatology, and gynecology. They often have the latest treatment options available.

3. Health Insurance Coverage: Check with your health insurance provider regarding coverage for HPV-related treatments. Many hospitals will accept national health insurance for standard procedures, but it's essential to confirm beforehand.

4. Follow-Up Care: After initial treatment, regular follow-up appointments are crucial to monitor the effectiveness of the treatment and to check for any recurrence of warts or dysplasia.

Conclusion
While the diagnosis of HPV and related conditions can be concerning, it is important to remember that many cases can be effectively managed with appropriate treatment and regular monitoring. Engaging with healthcare professionals who specialize in HPV will provide the best guidance tailored to your and your girlfriend's specific situations. Additionally, maintaining open communication with each other about health concerns and treatment options will help alleviate anxiety and promote better health outcomes.
