The color of the glans is unusual?

Hello Doctor: I have a question that I've been keeping to myself for a long time.
When I was about seventeen, during a family gathering over the New Year, I didn't clean myself for three days.
When I got home, I noticed that the glans was very itchy.
Since that time, I have observed that part of the glans has a different color than the usual light red; it appears to be somewhat white.
The area has expanded, and now it feels like most of it has turned white.
When not erect, the skin of the glans is no longer smooth as it used to be, but rather wrinkled, as if there is an additional layer of membrane that cannot be removed.
I have seen a doctor before, who diagnosed me with balanitis and prescribed an ointment.
After using it for two weeks without any improvement, I stopped paying attention to it.
It remains the same now, but it is not painful or itchy; it just has an unusual color and appears wrinkled when not erect.
I am unsure what this condition might be.
(P.S.
I have never engaged in sexual intercourse, but I do have a habit of masturbation, and I had a circumcision when I was younger.) I wonder if the doctor has encountered such clinical symptoms and could provide me with some insight.
Thank you very much!

Ha! No worries, that's just smegma, which is secreted by the sebaceous glands of the glans and foreskin.
Just make sure to wash your penis daily to keep it clean.
If there's itching, it could be eczema or a fungal infection, so please visit a urologist for an evaluation!

Understanding unusual changes in the color of the glans (the head of the penis) can be concerning, especially when these changes are accompanied by alterations in texture or appearance. Based on your description, it seems that you have experienced a change in the color of your glans from its normal reddish hue to a whitish appearance, along with a wrinkled texture when not erect.
Firstly, it is important to note that changes in the color and texture of the glans can be attributed to a variety of causes. One common condition that might explain your symptoms is balanitis, which is an inflammation of the glans. Balanitis can occur due to poor hygiene, infections (bacterial or fungal), or irritants such as soaps or lotions. In your case, the initial incident of not cleaning the area for an extended period may have contributed to the development of this condition.

The whitish appearance you describe could also be indicative of a condition known as lichen sclerosus, which is a chronic inflammatory skin condition that can affect the genital area. This condition often presents with white patches and can lead to changes in texture, including wrinkling. It is important to differentiate between these conditions, as they may require different treatment approaches.

Since you mentioned that you have previously seen a doctor who diagnosed you with balanitis and prescribed a topical ointment, it is concerning that you did not see any improvement after two weeks of treatment. If the condition persists without pain or discomfort, it may still warrant further investigation. Sometimes, a more potent topical steroid or antifungal medication may be necessary, depending on the underlying cause.

Additionally, it is worth considering that the wrinkled appearance of the glans when not erect could be a normal anatomical variation or a result of the skin's response to inflammation. However, if this change is new and persistent, it is advisable to seek a second opinion from a urologist or dermatologist who specializes in genital skin conditions. They can perform a thorough examination and may suggest additional tests, such as a biopsy, to rule out any serious conditions.

In terms of hygiene, maintaining proper cleanliness is crucial, especially in the genital area. Regular washing with mild soap and water can help prevent infections and irritations. It is also important to avoid using harsh soaps or products that may irritate the sensitive skin of the glans.

Lastly, while you mentioned that you have not engaged in sexual intercourse, it is still essential to practice safe hygiene habits, especially if you are engaging in masturbation. Using lubrication can help reduce friction and irritation during such activities.

In conclusion, while the changes you are experiencing may not be immediately alarming, they do warrant further investigation to ensure that there is no underlying condition that requires treatment. Seeking a second opinion and maintaining good hygiene practices will be beneficial in managing your symptoms and alleviating your concerns. Remember, it is always better to address these issues sooner rather than later to ensure your overall health and well-being.
